Glaucoma - An overview :

As per a report published in journal of ophthalmology - 2014 edition, 80 million people in the world were affected with glaucoma, the estimation is expected to rise to a whopping 111 million by the year 2040.

  • Glaucoma induced blindness or reduced vision affects 3.3 million Americans aged 40 and above.
  • Glaucoma is a leading cause of blindness in African Americans and Hispanics.
  • The chances of Glaucoma rises rapidly in people of Hispanic origin after 65 years of age.
  • As per 2020 ARVO Annual Meeting, Prevalence of Glaucoma in Mawanza region of northern Tanzania is quite higher as compared to rest of Tanzania.
  • As per estimation currently Nigeria may be having approx. 1.2 million people affected with Glaucoma. Prevalence is more in men as compared to women and IGBO ethnicity of Nigerians are more prone to glaucoma as compared to other tribes and ethnicity.
  • Glaucoma is quite a common eye disease in many African countries like Nigeria , Tanzania , Kenya , Uganda , Angola , Ghana , Zimbabwe and Zambia.
  • Almost 50% of Glaucoma patients are unaware that they are affected by it, this rises to almost 90% in underdeveloped and developing countries.
  • Unfortunately many individuals are unaware about glaucoma and it's very serious repercussions.
  • Many times Glaucoma is asymptomatic , that means the affected person does not exhibit or feel any symptoms indicating that he/she is affected by glaucoma.
  • An increased awareness about glaucoma can reduce visual disability ( blindness) for many people from all across the world.
  • Although there is still no cure for glaucoma but in many cases of glaucoma, blindness can be prevented with timely appropriate treatments.

Glaucoma patients from all across the world including Nigeria , Tanzania , Kenya , Zambia , Zimbabwe , Ghana , Uganda , Madagascar, Iraq , Oman , Bahrain, Kuwait, Bangladesh , Fiji, Maldives, Mauritius,Uzbekistan , Tajikistan , Kyrgyzstan, Turkmenistan, Europe and Australia come to India for cost effective glaucoma surgery in India. Types of glaucoma :

There are generally two types of glaucoma. But there are many other types of glaucoma too. They are usually variations of the two main types of glaucoma. The common thing between all types of glaucoma is increased intra-ocular pressure and as a consequence damage to optic nerve.

1. Open angle glaucoma :

The most common type of glaucoma is open angle glaucoma, it accounts to almost 90 % of all glaucoma cases. In open angle glaucoma cases eyes drainage canal gets clogged overtime. The clogged drainage canal increases intraocular pressure and damage the optic nerve overtime. It is a very slow process and the reason is still unknown. Most people with open angle glaucoma have no symptoms and no early warning signs. If it is not diagnosed early, it can cause a gradual loss of vision. Open angle glaucoma usually responds well to medication, especially if diagnosed early and treated in time.

Open-angle glaucoma is hereditical. People with either parents or grandparents diagnosed with open-angle glaucoma are at a higher risk of developing this type of glaucoma. People of African descent are also at higher risk of developing open angle glaucoma. Uncontrolled high blood pressure or diabetes can also be factors for this type of glaucoma.

2. Closed angle glaucoma :

Close angle glaucoma or angle closure glaucoma are also known as acute glaucoma or narrow angle glaucoma. Close angle glaucoma is a serious medical condition and need prompt medical management as there may be a sharp rise in intraocular pressure that can be detrimental to optic nerve.

In angle closure glaucoma pupil in eye enlarges too much and swiftly. The enlarged pupil's outer edges of the iris blocks the drainage canal. When the fluid in the eye is not drained out smoothly because of blocked drainage canal there is a sudden increase in intraocular pressure. The sudden and severe increase in intraocular pressure damages the optic nerve.

Although this type of glaucoma can occur in any person but it affects women more than men. People of Chinese origin are more prone to get this type of glaucoma.

When angle closure glaucoma progresses it is termed as slowly and without any noticeable symptoms , it is termed as chronic angle closure glaucoma.

The various known types of Glaucoma ( usually variations of two main types - Open angle glaucoma and angle closure glaucoma ) are listed here :

Normal tension glaucoma :

Normal tension glaucoma is a subtype of open angle glaucoma, it occurs in people even with normal intraocular pressure. It is also called low tension or normal pressure glaucoma and the optic nerve is damaged even when the intraocular pressure is within the normal range. Normal tension glaucoma is associated with migraine, low blood pressure or irregular heart beats. Factors responsible for normal tension glaucoma include abnormalities in the blood flow to the optic nerve, and structural weakness of the optic nerve tissue. Family history is also a causative factor as well as ethnicity. People of Japanese origin are more prone to having normal tension glaucoma.

Congenital glaucoma:

Congenital glaucoma is the type of glaucoma with which a child is born. It May develop in children because of ill formed or under developed internal drainage system in eye during the pregnancy. In most of the cases , congenital glaucoma is diagnosed by the age of six months, with 80% of all childhood glaucoma are diagnosed in the first year of life of the child

Congenital glaucoma may cause blindness in child during his growing days. Consultation with a glaucoma specialist should be arranged as soon as possible and periodic follow-up is necessary for checking up of intra-ocular pressure and optic nerve. Eye glasses and eye-patching might be necessary for the development and maintenance of a good vision.

Juvenile glaucoma :

Juvenile glaucoma affects teenagers or young adults. The signs and symptoms of Juvenile glaucoma are similar to adult glaucoma. Juvenile glaucoma is considered more risky for developing blindness because the chances of intraocular pressure being high is more in this age group.

As the onset of disease sets in at an early age , the the need to protect and preserve the optic nerve is for a longer period of time. Although juvenile glaucoma is not a very common disease but since the higher risk of blindness is involved periodic testing of intraocular pressure and optical disc evaluation is a must for juvenile glaucoma.

Secondary glaucoma :

Secondary glaucoma can be both open angle glaucoma or angle closure glaucoma. With these types of glaucoma the underlying cause can be identified. Secondary glaucoma may take place as a result of an eye injury, inflammation, certain drugs such as steroids and advanced cases of cataract or uncontrolled prolonged diabetes. Secondary glaucoma can also occur in one or both eyes. There are various types of secondary glaucoma , depending upon the underlying identified cause.

Neovascular glaucoma :

Neovascular glaucoma is secondary angle closure glaucoma. Neovascular glaucoma is an aggressive form of glaucoma and usually occur in people with retinal vascular occlusion or uncontrolled diabetic retinopathy. Retinal ischemia takes place in such patients which produces angiogenic factors. These factors create new vessels at iris pupillary border and over the eye's aqueous humor drainage channels. It raises intraocular pressure and damage the optic nerve. This complex glaucoma treatment often requires the efforts of both glaucoma and retina specialists. Diabetic patients need to control diabetes to avoid such type of glaucoma.

Exfoliative Glaucoma :

Exfoliative glaucoma is a type of open angle glaucoma, characterized by peeling off a flaky, dandruff-like material from the outer layer of the lens within the eye. It collects in the angle between the cornea and iris and can clog the drainage canals of the eye, causing intraocular pressure to rise and damage the optic nerve. Exfoliative glaucoma type of glaucoma progresses faster than open-angle glaucoma, and often causes higher eye pressure. Therefore it is important for people who are at risk of developing exfoliative glaucoma to get regular eye exams to protect their vision.

Pigmentary Glaucoma :

Pigmentary glaucoma occurs because of flaking off pigment granules in Iris and blocking the drainage channels of aqua humorous fluid in eye. The blocked drainage channels increases intraocular pressure and damage the optic nerve.

Traumatic glaucoma :

Any trauma to eye may trigger traumatic glaucoma. It may occur immediately after trauma or may take years to develop after the trauma to eye. It is a form of open angle glaucoma and may occur due to blunt injuries that bruise the eye (called blunt trauma) or by injuries that penetrate the eye.

Uveitic Glaucoma:

Uveitis is a medical condition that causes swelling and inflammation in middle layer of eye. 2 out of 10 people who have uveitis are prone to developing uveitic Glaucoma. Steroids prescribed for treating uveitis may also cause uveitic glaucoma. It is most commonly noticed with corticosteroids. It can occur due to usage of steroid eye drops taken for allergic conjunctivitis/uveitis. Even the use of steroid containing inhalers (taken by patients who suffer from asthma), nasal spray, skin ointments and any oral or IV medication containing steroids may cause this type of glaucoma. What is noteworthy is that, not only corticosteroids may cause glaucoma, but their usage may also lead to loss of Intraocular pressure control in patients who are on glaucoma treating medications.

Glaucoma signs and symptoms :

Most of the times glaucoma is asymptomatic. The most prevalent type of glaucoma is primary open glaucoma. In primary open glaucoma the intraocular pressure buildup is slowly progressive and therefore the initial stages are asymptomatic. In other but less common types of glaucoma like the angle closure glaucoma the following signs and symptoms can indicate presence of glaucoma in a person.

  • Hazy vision. In the initial stages it is only the peripheral vision that is hazy but in the advance stages , even the center vision is also blurred.
  • redness of eye
  • headache
  • intense eye pain
  • Blurry vision
  • Nausea or feeling of emesis
  • seeing rings around the source of light
  • seeing rainbow coloured lights at a light source.

Risk factors for Glaucoma:

Glaucoma can affect people of all ages . A new born child may have born with congenital glaucoma , young adults or adolescents may suffer from Juvenile glaucoma. So almost people of all ages are at risk of developing glaucoma. The identified risk factors for glaucoma are :

Advancing age :

Although glaucoma can affect people of all ages but advancing age is identified as a possible causative factor for glaucoma. People above 40 years of age must undergo periodic eye screening for ruling out onset of glaucoma.

Genetic factor :

People with close relatives diagnosed with glaucoma are more prone to get affected by it.

Ethnicity as causative risk factor for glaucoma :

People of certain ethnicities are prone to acquire certain types of glaucoma.

  • Having an African ancestry puts one at risk of developing open angle glaucoma
  • people with Chinese origin are more prone to closed angle glaucoma
  • people with Japanese origin are more prone to be affected with normal tension glaucoma

Astigmatism or vision problems:

People who are very near sighted ( myopia) are very far sighted (hyperopia) stand higher risk of developing glaucoma.

Usage of corticosteroid medications:

An eye specialist may prescribe certain eye drops containing steroids for treatment of eye medical conditions like uveitis or conjunctivitis may increase the risk of glaucoma. Asthma inhalers containing steroids may have the same effects as well as tropical medication like ointments or cream for skin conditions may also increase the risk of glaucoma.

Thin central cornea:

A thin central corneal thickness is identifies as a potential risk factor for glaucoma. Thinner corneas are associated with an increased severity of visual field loss and a more rapid progression of the disease.

Trauma to eye:

An injury to eye either external or external increases risk of developing glaucoma.

Diagnosis of Glaucoma in India :

Best Glaucoma Surgery hospitals in India perform following investigations for accurate diagnosis of glaucoma. These tests also help top glaucoma Surgeons in India in determining the type of glaucoma and subsequently the appropriate treatment.

Tonometry for measuring intraocular pressure:

Intraocular pressure is measured with an instrument called tonometer. Tonometry can be done by a non-contact or contact method.

  • Contact Tonometry is done under local anesthesia where doctors put eye drops to numb the cornea. Tonometer touches the tip of cornea and measures the intraocular pressure.
  • The air puff or noncontact tonometer emits a puff of air. The test measures eye pressure by the eye's resistance to the air.

Ophthalmoscopy for optic nerve examination:

After dilating the pupil of eye , doctors will use special lenses to look inside the eye and at optic nerve head for assessing the shape and color of optic nerve. Doctors will also examine the optic nerve for the presence of any glaucomatous structural damage. The procedure is swift and painless.

Doctors performing Ophthalmoscopy will also take images of optic nerve to record optic nerve changes overtime. Imaging optic disc over time during visits to ophthalmologists can help in monitoring any progressive loss of optic nerve fibers. Commonly used imaging tests are:

  • Stereo optic nerve photographs
  • Scanning laser polarimetry (GDx)
  • Confocal scanning laser ophthalmoscopy (Heidelberg Retinal Tomograph or HRT)
  • Optical coherence tomography (OCT)

All the above mentioned imaging tests are non invasive and painless. Optical coherence tomography (OCT) is the most commonly used. OCT measures the reflection of laser light (much like ultrasound measures the reflection of sound) and can directly measure the thickness of the nerve fiber layer.

Perimetry for visual field monitoring :

Monitoring the visual field for glaucoma Surgery in India with top glaucoma surgeons at best eye surgery hospitals involves automatic perimetry. Automatic perimetry is the reference standard exam to assess visual function. The tests are done for monitoring any changes in peripheral as well as central visual field.

During Perimetry , one has to look straight ahead and then press a button whenever one sees a spot of light in the peripheral (side) vision. The spots of light will vary in intensity such that some are very easy to see and others are not visible even when vision is normal.

As Glaucoma tends to affect the peripheral visual field first and gradually affecting the central visual field in advance stages. The Perimetry or visual field tests is repeated twice or at least once a year to ascertain any disease progression.

Gonioscopy for determining Glaucoma type:

Gonioscopy is the reference standard examination of the eye to evaluate the angle – where the drainage system of the eye is located. Procedure is done under local anesthesia to avoid any discomfort to patient. It is a painless procedure to determine if the glaucoma is angle closure glaucoma or open angle glaucoma.

Pachymetry for determining Corneal thickness :

Pachymetry is done for assessing the thickness of cornea. A thin cornea is a risk factor for glaucoma. Corneal thickness can influence eye pressure readings. If cornea is thicker than average, pressure readings with a tonometer may be higher than the actual pressure and if a cornea is thinner pressure readings may be lower than actual. Pachymetry is painless test done by using a placing a probe gently on the eye. For glaucoma treatment in India with top doctors at best hospitals , pachymetry is done to determine the treatment course.

Glaucoma surgery in India:

The objective of glaucoma treatment is to reduce intraocular pressure and preserve the optic disc. At CMCS health , we assure best and cost effective glaucoma surgery in India with top doctors at best glaucoma hospitals. Know about Glaucoma surgery cost in India with Top Indian glaucoma doctors.Glaucoma surgery can be done in two ways:

1. Non Incisional laser glaucoma surgery in India:

They are laser procedures with no actual surgery or cut made in the eye. They are done by treating the medical condition with the help of lasers and as a day care procedures performed under the local anesthesia. Non Incisional laser glaucoma surgery cost in India at our associated best eye surgery hospitals in India are lowest.

Best Laser Glaucoma surgery in India :

Laser treatments are being used worldview for control and treatment of Glaucoma. Glaucoma is caused by increased intraocular pressure due to excessive fluid retention in eyes and blocked or obstructed fluid drainage channels . The objectives of laser procedures to treat glaucoma is to reduce the amount of fluid in the eye, either by opening the blocked or obstructed drainage channels, or by reducing the amount of fluid that is made in the eye. Laser glaucoma surgery are more popular glaucoma surgery currently. Laser surgery are performed in a day care setup that means there is no need of hospital admission for the patient. Laser glaucoma surgery can be done for both open angle glaucoma and angle closure glaucoma. Laser glaucoma surgeries are performed to lower the intraocular pressure ( IOL) and relieve the pressure on optic nerve. The length of time for which the IOL remains low depend on following factors.

  • Age of the patient
  • Type of glaucoma
  • Other medical conditions

Many patients who have undergone laser glaucoma surgery may still need the medications for lowing IOL but the dosage may be lowered.

Following Glaucoma laser surgery in India are performed at best eye surgery hospitals in India by top glaucoma surgeons.

Laser Trabeculoplasty in India :

It is a laser procedure and first line treatment for open angle glaucoma. Laser trabeculoplasty can be performed either by selective laser trabeculoplasty ( SLT ) or Argon laser trabeculoplasty ( ALT ) . In this procedure laser is targeted at drainage canals in eye and open it so the fluid can drain out smoothly and the intraocular pressure is reduced thereby preventing the optic nerve from damage. Procedure is done under local anesthesia. Patient may be prescribed some eye drops to reduce inflammation and swelling. Both SLT and ALT are similar but SLT is a newer type that uses gentler laser.

Laser peripheral Iridotomy or iridoplasty in India :

Laser peripheral iridotomy or iridoplasty is a standard first line treatment for angle closure glaucoma. In laser iridoplasty, the laser is targeted at the iris to make a hole in Iris to bring it back to it's original position. It opens up the drainage channel from posterior chamber into the anterior chamber by bypassing the pupillary block.

Laser cyclophotocoagulation glaucoma surgery in India :

Laser cyclophotocoagulation glaucoma surgery is done by using Cyclodiode laser. The laser targets the ciliary body. Ciliary body are the structures in eye that produces the aqueous humour. Objective of treatment is to reduce the production of aqueous humour. Treatment is done as a day care procedure under local anesthesia.

Best Glaucoma hospitals in India uses the latest MicroPulse cyclodiode device. It works in the same way as conventional cyclodiode laser device but uses short bursts of laser targeting the ciliary body. It allows the ciliary body to cool between pulses and reduce the damage to the surrounding tissue.

2. Incisional glaucoma Surgery in India :

Incisional Glaucoma surgery is done when the medications and glaucoma laser surgery fail to control the intraocular pressure and there is an evident risk of damage to optical nerve. Glaucoma incisional Surgery is also indicated for those glaucoma patients who have allergy to glaucoma medications, are not compliant to therapy or the disease continues to progress despite treatment. Advanced disease and other conditions are also indication for surgery. At best glaucoma hospitals , the top Glaucoma specialist doctors will discuss the best possible treatment options with the patients. While deciding about the Glaucoma incisional surgery and it's type, top glaucoma doctors consider the following factors:

  • The severity of the disease
  • how the patient has responded to glaucoma medications
  • Other underlying medical conditions and health issues with patient

The common incisional glaucoma surgeries done in India by the top glaucoma Surgeons at best glaucoma hospitals are listed below:

Incisional trabeculectomy glaucoma surgery in India:

The most commonly performed incisional glaucoma surgery in the world for treating glaucoma is trabeculectomy surgery. A small guarded fistula ( hole ) is made in the wall of the eyeball at the level of the junction of the cornea and the sclera. This guarded fistula allow the egress of the aqueous humor through a new alternative outflow way. Excessive aqueous humor is collected in a reservoir at the sub-Tenon/sub-conjunctival space that is called filtering bleb. Incisional trabeculectomy glaucoma surgery is generally done under the superior eyelid. The sutures control the amount of aqueous humor flow. From the bleb, the drained aqueous humor is then absorbed through the capillaries to the blood circulation and drained out of the eye. A small white coloured blister / scar can be seen in the upper part of the eye and is called a “bleb”. In most cases, the bleb remains covered by the superior eyelid and cannot be seen unless the eyelid is retracted.

A variation to trabeculectomy glaucoma surgery is to create a bleb with an Ex-Press shunt. The metal shunt does not affect MRI scans or airport security. Advantages of placing the shunt is that it hastens the vision recovery and less inflammation inside the eye after surgery.

Aqueous shunt or drainage implant glaucoma surgery in India :

An aqueous shunt or drainage implant glaucoma surgery involves putting a tube, shunt or other devices to create a new drainage channel. An implant is advised usually when other types of eye surgery fail to reduce the intraocular pressure. The aqueous shunt or implants consist of a silicon tube, which is connected to a draining plate. The tube is inserted in the eye and the drainage plate is sutured to sclera. The excessive aqueous humor is drained through or along the tube or shunt and collects around the plate at the back of the eye, where it is gradually absorbed by capillaries and the lymphatic system and transported out of the eye.

Minimally invasive incisional glaucoma surgery ( MIGS ) in India :

The minimally invasive incisional glaucoma surgery is an innovation or upgradation to conventional incisional glaucoma surgeries. With this new glaucoma surgery technique , surgery can be done earlier for glaucoma patients and more safely than conventional surgery.

Minimally invasive glaucoma surgery involves using smaller incisions and/or microscopic equipment, which have been designed to reduce risk associated with conventional trabeculectomy glaucoma surgery or the Aqueous shunt or drainage implant glaucoma surgery.

Glaucoma surgery for infants in India :

Two main types of surgery are done in India for infants with congenital glaucoma.

  • Goniotomy is used almost exclusively worldwide for infants with congenital glaucoma. In Goniotomy procedure, the doctor inserts a tiny blade through the cornea to cut the trabecular meshwork. This procedure allows the eye fluid to flow normally out of the eye.
  • Trabeculectomy is another glaucoma surgery used for infants with congenital glaucoma. This surgery involves making an incision in the outer portion of the eye and using a tiny probe to break through the trabecular meshwork. Aqueous humor is then able to drain out of the eye, keeping eye pressure in a normal range.
