Etiology of Cataract formation :

When the light falls on the clear and transparent , natural lens in eye , it reaches retina. Retina converts this light into nerve signals. These nerve signals are then sent to brain for visual interpretation. Because of the natural eye lens becoming opaque ( Cataract ) , complete light does not reaches retina. As a consequence the nerve signals to brain are not proper. Improper nerve signals to brain causes poor visual interpretation. The cloudy patches or opaqueness in the otherwise clear and transparent natural eye lens is formation of cataract.

Reasons for formation of cataract :

The natural lens in eye is composed of proteins and water. These proteins keep the lens clear and transparent. Because of normal ageing, these proteins can start to degrade, causing a small cloudy patch to develop in the otherwise transparent lens. Identified factors for cataract formation are :

  • Degeneration of proteins that are supposed to keep the lens clear and transparent because of normal ageing.
  • Other medical conditions predisposing to cataract are diabetes, hypertension, lack of vitamins (vitamins C and E, selenium, beta-carotene, and lycopene), sun exposure for long duration, injury to eye, smoking, chronic kidney disease, an infection or swelling in the eye.
  • Prolonged usage of certain medications like sedatives ( chlorpromazine ) , cardiac medication ( amiodarone ) , epilepsy medication ( phenytoin) , cholesterol lowering medication ( lovastatin ) and corticosteroids ( prednisolone ).

Infants or young children may also develop cataract. Possible factors responsible for formation of cataract in infants ( congenital ) or young children could be :

Genetic factors:

Family disposition is a cause for congenital cataracts in around 1 out of every 5 infants. Following genetic mutations may cause cataract in infants:

  • Alport's syndrome.
  • Cri-du-chat syndrome.
  • Patau's syndrome.
  • Trisomy 18 (Edward's syndrome)
  • Turner's syndrome
  • Down's Syndrome.

Infection to mother :

Congenital cataract chances increases in an infant if during the pregnancy, mother is infected with:

  • Rubella virus
  • Toxoplasmosis, which is a parasitic infected acquired by consuming food , water or soil contaminated by the feces of an infected cat.
  • Cytomegalovirus ( CMV ) a common virus.
  • Chickenpox infection
  • Herpes simplex infection.

Symptoms of cataract :

As cataract develops slowly , the symptoms are not observed in early stages. when the cataract grows the affected person may observe following symptoms:

  • Eye lens appears Cloudy.
  • Blurred vision.
  • Double vision
  • Increased sensitivity to glare.
  • Trouble in night vision.
  • Halos surrounding the source of light.
  • Frequent changes in refractive error and subsequent changes in eye glasses or contact lens numbers.
  • Fading of colours or yellowish tint.

Types of cataract :

Cataracts can be classified depending on their etiology and location. The most common types of cataract are :

Nuclear cataract:

Cataracts that start in the centre of lens , it is called a nuclear cataract. A nuclear cataract may cause temporary improvement is reading vision. But gradually the lens will turns yellowish or in advance stage brownish. This discolouration of lens will make a cataract person less susceptible to actual color of objects.

Cortical cataracts:

Cortical cataracts forms at the edges of the lens and gradually progress to the center of lens. The formation of cortical cataracts is due to a previous eye injury, inflammation, aging, and a family history of cataracts. It can be both anterior cortical cataract as well as posterior cortical cataract.

Subcapsular Cataract :

Like all cataracts, subcapsular cataracts are formed because of normal ageing, but these cataracts can also occur after trauma, long duration of treatment with corticosteroids, exposure to ionizing radiation, excessive alcohol consumption, and inflammation. Reading vision is affected because of posterior subcapsular cataract. It also reduces vision in bright light, and causes glare or halos around lights at night. These types of cataracts tend to progress faster than other types do.

Congenital cataracts:

in congenital cataract a child is born with cataract. These cataract are also termed as infantile cataracts.

Cataract diagnosis in India :

When a patient visit an eye doctor with complaints of having symptoms related to cataract. The treating doctor will take a family history of patient , do physical examination of eyes and prescribe following investigations to conform the diagnosis.

Visual acuity tests :

In a visual acuity test doctor or eye technician will ask the patient to read alphabets on a chart or a reading device. The Alphabets are arranged in a progressively deceasing size. Optometrist or eye technician will cover one of the eye and ask the patient to read the alphabets on the device or chart. Starting form the bigger sizes to smallest. The test is done for one eye at a time for both the eyes. The Visual acuity test will help the optometrist in determining if the patient is having a normal 20/20 vision in both eyes or an impaired vision in either of the eyes.

Slit lamp examination :

For detecting any abnormality in the structures of eye including cornea, iris, lens, and the space between iris and cornea, eye specialist doctors will advise for a slit lamp examination. Patient will be asked to sit on an examination chair, in front of a microscope. A beam of light ( slit lamp ) is directed on the pupil and the microscope is used to magnify the inner structures of eye for observing any abnormality. The slit allows the doctor to view eye structures in small sections and makes it easy to detect any abnormalities.

Retina examination :

Retina co ordinates between the light that enters the eyes and the images that one sees. Doctors may do retina examination by dilating the pupil with eye drops. Once the pupil are dilated, doctor will then use a slit-lamp or ophthalmoscope to observe if the lens is being affected by a cataract. Although it is difficult to see retina when cataract is formed in the eye.

Phacoemulsification cataract surgery in India :

Phacoemulsification involves making a very small incision in the side of cornea. With this very small incision in eye, surgeon will insert a probe connected to a Phaco machine and breaks the opaque lens of eye with ultrasonic waves. Once the opaque lens is converted to an emulsified gelatinous mass, it is flushed with sterile fluids (irrigation) and suctioned out of the eye with a vacuum. An artificial foldable Intraocular lens is then placed inside the eye and the very small incision is closed with very small amount of saline to swell the surrounding cornea and seal the opening.

Most of the modern cataract surgeries are done by this minimally invasive technique. Phacoemulsification is done by a highly sophisticated device, a phaco machine. This machine consists of a handpiece, foot pedal, irrigation and aspiration system, and different types of tips that are bent at different angles to accurately deliver the ultrasonic waves at the opaque eye lens. For cost effective phacoemulsification cataract removal surgery in India , send your medical queries to us.

Glaucoma - An overview :

As per a report published in journal of ophthalmology - 2014 edition, 80 million people in the world were affected with glaucoma, the estimation is expected to rise to a whopping 111 million by the year 2040.

  • Glaucoma induced blindness or reduced vision affects 3.3 million Americans aged 40 and above.
  • Glaucoma is a leading cause of blindness in African Americans and Hispanics.
  • The chances of Glaucoma rises rapidly in people of Hispanic origin after 65 years of age.
  • As per 2020 ARVO Annual Meeting, Prevalence of Glaucoma in Mawanza region of northern Tanzania is quite higher as compared to rest of Tanzania.
  • As per estimation currently Nigeria may be having approx. 1.2 million people affected with Glaucoma. Prevalence is more in men as compared to women and IGBO ethnicity of Nigerians are more prone to glaucoma as compared to other tribes and ethnicity.
  • Glaucoma is quite a common eye disease in many African countries like Nigeria , Tanzania , Kenya , Uganda , Angola , Ghana , Zimbabwe and Zambia.
  • Almost 50% of Glaucoma patients are unaware that they are affected by it, this rises to almost 90% in underdeveloped and developing countries.
  • Unfortunately many individuals are unaware about glaucoma and it's very serious repercussions.
  • Many times Glaucoma is asymptomatic , that means the affected person does not exhibit or feel any symptoms indicating that he/she is affected by glaucoma.
  • An increased awareness about glaucoma can reduce visual disability ( blindness) for many people from all across the world.
  • Although there is still no cure for glaucoma but in many cases of glaucoma, blindness can be prevented with timely appropriate treatments.

Types of glaucoma :

There are generally two types of glaucoma. But there are many other types of glaucoma too. They are usually variations of the two main types of glaucoma. The common thing between all types of glaucoma is increased intra-ocular pressure and as a consequence damage to optic nerve.

1. Open angle glaucoma :

The most common type of glaucoma is open angle glaucoma, it accounts to almost 90 % of all glaucoma cases. In open angle glaucoma cases eyes drainage canal gets clogged overtime. The clogged drainage canal increases intraocular pressure and damage the optic nerve overtime. It is a very slow process and the reason is still unknown. Most people with open angle glaucoma have no symptoms and no early warning signs. If it is not diagnosed early, it can cause a gradual loss of vision. Open angle glaucoma usually responds well to medication, especially if diagnosed early and treated in time.

Open-angle glaucoma is hereditical. People with either parents or grandparents diagnosed with open-angle glaucoma are at a higher risk of developing this type of glaucoma. People of African descent are also at higher risk of developing open angle glaucoma. Uncontrolled high blood pressure or diabetes can also be factors for this type of glaucoma.

2. Closed angle glaucoma :

Close angle glaucoma or angle closure glaucoma are also known as acute glaucoma or narrow angle glaucoma. Close angle glaucoma is a serious medical condition and need prompt medical management as there may be a sharp rise in intraocular pressure that can be detrimental to optic nerve.

In angle closure glaucoma pupil in eye enlarges too much and swiftly. The enlarged pupil's outer edges of the iris blocks the drainage canal. When the fluid in the eye is not drained out smoothly because of blocked drainage canal there is a sudden increase in intraocular pressure. The sudden and severe increase in intraocular pressure damages the optic nerve.

Although this type of glaucoma can occur in any person but it affects women more than men. People of Chinese origin are more prone to get this type of glaucoma.

When angle closure glaucoma progresses it is termed as slowly and without any noticeable symptoms , it is termed as chronic angle closure glaucoma.

The various known types of Glaucoma ( usually variations of two main types - Open angle glaucoma and angle closure glaucoma ) are listed here :

Normal tension glaucoma :

Normal tension glaucoma is a subtype of open angle glaucoma, it occurs in people even with normal intraocular pressure. It is also called low tension or normal pressure glaucoma and the optic nerve is damaged even when the intraocular pressure is within the normal range. Normal tension glaucoma is associated with migraine, low blood pressure or irregular heart beats. Factors responsible for normal tension glaucoma include abnormalities in the blood flow to the optic nerve, and structural weakness of the optic nerve tissue. Family history is also a causative factor as well as ethnicity. People of Japanese origin are more prone to having normal tension glaucoma.

Congenital glaucoma:

Congenital glaucoma is the type of glaucoma with which a child is born. It May develop in children because of ill formed or under developed internal drainage system in eye during the pregnancy. In most of the cases , congenital glaucoma is diagnosed by the age of six months, with 80% of all childhood glaucoma are diagnosed in the first year of life of the child

Congenital glaucoma may cause blindness in child during his growing days. Consultation with a glaucoma specialist should be arranged as soon as possible and periodic follow-up is necessary for checking up of intra-ocular pressure and optic nerve. Eye glasses and eye-patching might be necessary for the development and maintenance of a good vision.

Juvenile glaucoma :

Juvenile glaucoma affects teenagers or young adults. The signs and symptoms of Juvenile glaucoma are similar to adult glaucoma. Juvenile glaucoma is considered more risky for developing blindness because the chances of intraocular pressure being high is more in this age group.

As the onset of disease sets in at an early age , the the need to protect and preserve the optic nerve is for a longer period of time. Although juvenile glaucoma is not a very common disease but since the higher risk of blindness is involved periodic testing of intraocular pressure and optical disc evaluation is a must for juvenile glaucoma.

Secondary glaucoma :

Secondary glaucoma can be both open angle glaucoma or angle closure glaucoma. With these types of glaucoma the underlying cause can be identified. Secondary glaucoma may take place as a result of an eye injury, inflammation, certain drugs such as steroids and advanced cases of cataract or uncontrolled prolonged diabetes. Secondary glaucoma can also occur in one or both eyes. There are various types of secondary glaucoma , depending upon the underlying identified cause.

Neovascular glaucoma :

Neovascular glaucoma is secondary angle closure glaucoma. Neovascular glaucoma is an aggressive form of glaucoma and usually occur in people with retinal vascular occlusion or uncontrolled diabetic retinopathy. Retinal ischemia takes place in such patients which produces angiogenic factors. These factors create new vessels at iris pupillary border and over the eye's aqueous humor drainage channels. It raises intraocular pressure and damage the optic nerve. This complex glaucoma treatment often requires the efforts of both glaucoma and retina specialists. Diabetic patients need to control diabetes to avoid such type of glaucoma.

Exfoliative Glaucoma :

Exfoliative glaucoma is a type of open angle glaucoma, characterized by peeling off a flaky, dandruff-like material from the outer layer of the lens within the eye. It collects in the angle between the cornea and iris and can clog the drainage canals of the eye, causing intraocular pressure to rise and damage the optic nerve. Exfoliative glaucoma type of glaucoma progresses faster than open-angle glaucoma, and often causes higher eye pressure. Therefore it is important for people who are at risk of developing exfoliative glaucoma to get regular eye exams to protect their vision.

Pigmentary Glaucoma :

Pigmentary glaucoma occurs because of flaking off pigment granules in Iris and blocking the drainage channels of aqua humorous fluid in eye. The blocked drainage channels increases intraocular pressure and damage the optic nerve.

Traumatic glaucoma :

Any trauma to eye may trigger traumatic glaucoma. It may occur immediately after trauma or may take years to develop after the trauma to eye. It is a form of open angle glaucoma and may occur due to blunt injuries that bruise the eye (called blunt trauma) or by injuries that penetrate the eye.

Uveitic Glaucoma:

Uveitis is a medical condition that causes swelling and inflammation in middle layer of eye. 2 out of 10 people who have uveitis are prone to developing uveitic Glaucoma. Steroids prescribed for treating uveitis may also cause uveitic glaucoma. It is most commonly noticed with corticosteroids. It can occur due to usage of steroid eye drops taken for allergic conjunctivitis/uveitis. Even the use of steroid containing inhalers (taken by patients who suffer from asthma), nasal spray, skin ointments and any oral or IV medication containing steroids may cause this type of glaucoma. What is noteworthy is that, not only corticosteroids may cause glaucoma, but their usage may also lead to loss of Intraocular pressure control in patients who are on glaucoma treating medications.

Glaucoma signs and symptoms :

Most of the times glaucoma is asymptomatic. The most prevalent type of glaucoma is primary open glaucoma. In primary open glaucoma the intraocular pressure buildup is slowly progressive and therefore the initial stages are asymptomatic. In other but less common types of glaucoma like the angle closure glaucoma the following signs and symptoms can indicate presence of glaucoma in a person.

  • Hazy vision. In the initial stages it is only the peripheral vision that is hazy but in the advance stages , even the center vision is also blurred.
  • redness of eye
  • headache
  • intense eye pain
  • Blurry vision
  • Nausea or feeling of emesis
  • seeing rings around the source of light
  • seeing rainbow coloured lights at a light source.

Risk factors for Glaucoma:

Glaucoma can affect people of all ages . A new born child may have born with congenital glaucoma , young adults or adolescents may suffer from Juvenile glaucoma. So almost people of all ages are at risk of developing glaucoma. The identified risk factors for glaucoma are :

Advancing age :

Although glaucoma can affect people of all ages but advancing age is identified as a possible causative factor for glaucoma. People above 40 years of age must undergo periodic eye screening for ruling out onset of glaucoma.

Genetic factor :

People with close relatives diagnosed with glaucoma are more prone to get affected by it.

Ethnicity as causative risk factor for glaucoma :

People of certain ethnicities are prone to acquire certain types of glaucoma.

  • Having an African ancestry puts one at risk of developing open angle glaucoma
  • people with Chinese origin are more prone to closed angle glaucoma
  • people with Japanese origin are more prone to be affected with normal tension glaucoma

Astigmatism or vision problems:

People who are very near sighted ( myopia) are very far sighted (hyperopia) stand higher risk of developing glaucoma.

Usage of corticosteroid medications:

An eye specialist may prescribe certain eye drops containing steroids for treatment of eye medical conditions like uveitis or conjunctivitis may increase the risk of glaucoma. Asthma inhalers containing steroids may have the same effects as well as tropical medication like ointments or cream for skin conditions may also increase the risk of glaucoma.

Thin central cornea:

A thin central corneal thickness is identifies as a potential risk factor for glaucoma. Thinner corneas are associated with an increased severity of visual field loss and a more rapid progression of the disease.

Trauma to eye:

An injury to eye either external or external increases risk of developing glaucoma.

Diagnosis of Glaucoma in India :

Best Glaucoma Surgery hospitals in India perform following investigations for accurate diagnosis of glaucoma. These tests also help top glaucoma Surgeons in India in determining the type of glaucoma and subsequently the appropriate treatment.

Tonometry for measuring intraocular pressure:

Intraocular pressure is measured with an instrument called tonometer. Tonometry can be done by a non-contact or contact method.

  • Contact Tonometry is done under local anesthesia where doctors put eye drops to numb the cornea. Tonometer touches the tip of cornea and measures the intraocular pressure.
  • The air puff or noncontact tonometer emits a puff of air. The test measures eye pressure by the eye's resistance to the air.

Ophthalmoscopy for optic nerve examination:

After dilating the pupil of eye , doctors will use special lenses to look inside the eye and at optic nerve head for assessing the shape and color of optic nerve. Doctors will also examine the optic nerve for the presence of any glaucomatous structural damage. The procedure is swift and painless.

Doctors performing Ophthalmoscopy will also take images of optic nerve to record optic nerve changes overtime. Imaging optic disc over time during visits to ophthalmologists can help in monitoring any progressive loss of optic nerve fibers. Commonly used imaging tests are:

  • Stereo optic nerve photographs
  • Scanning laser polarimetry (GDx)
  • Confocal scanning laser ophthalmoscopy (Heidelberg Retinal Tomograph or HRT)
  • Optical coherence tomography (OCT)

All the above mentioned imaging tests are non invasive and painless. Optical coherence tomography (OCT) is the most commonly used. OCT measures the reflection of laser light (much like ultrasound measures the reflection of sound) and can directly measure the thickness of the nerve fiber layer.

Perimetry for visual field monitoring :

Monitoring the visual field for glaucoma Surgery in India with top glaucoma surgeons at best eye surgery hospitals involves automatic perimetry. Automatic perimetry is the reference standard exam to assess visual function. The tests are done for monitoring any changes in peripheral as well as central visual field.

During Perimetry , one has to look straight ahead and then press a button whenever one sees a spot of light in the peripheral (side) vision. The spots of light will vary in intensity such that some are very easy to see and others are not visible even when vision is normal.

As Glaucoma tends to affect the peripheral visual field first and gradually affecting the central visual field in advance stages. The Perimetry or visual field tests is repeated twice or at least once a year to ascertain any disease progression.

Gonioscopy for determining Glaucoma type:

Gonioscopy is the reference standard examination of the eye to evaluate the angle – where the drainage system of the eye is located. Procedure is done under local anesthesia to avoid any discomfort to patient. It is a painless procedure to determine if the glaucoma is angle closure glaucoma or open angle glaucoma.

Pachymetry for determining Corneal thickness :

Pachymetry is done for assessing the thickness of cornea. A thin cornea is a risk factor for glaucoma. Corneal thickness can influence eye pressure readings. If cornea is thicker than average, pressure readings with a tonometer may be higher than the actual pressure and if a cornea is thinner pressure readings may be lower than actual. Pachymetry is painless test done by using a placing a probe gently on the eye. For glaucoma treatment in India with top doctors at best hospitals , pachymetry is done to determine the treatment course.

Glaucoma surgery in India:

The objective of glaucoma treatment is to reduce intraocular pressure and preserve the optic disc. At CMCS health , we assure best and cost effective glaucoma surgery in India with top doctors at best glaucoma hospitals. Know about Glaucoma surgery cost in India with Top Indian glaucoma doctors.Glaucoma surgery can be done in two ways:

1. Non Incisional laser glaucoma surgery in India:

They are laser procedures with no actual surgery or cut made in the eye. They are done by treating the medical condition with the help of lasers and as a day care procedures performed under the local anesthesia. Non Incisional laser glaucoma surgery cost in India at our associated best eye surgery hospitals in India are lowest.

Best Laser Glaucoma surgery in India :

Laser treatments are being used worldview for control and treatment of Glaucoma. Glaucoma is caused by increased intraocular pressure due to excessive fluid retention in eyes and blocked or obstructed fluid drainage channels . The objectives of laser procedures to treat glaucoma is to reduce the amount of fluid in the eye, either by opening the blocked or obstructed drainage channels, or by reducing the amount of fluid that is made in the eye. Laser glaucoma surgery are more popular glaucoma surgery currently. Laser surgery are performed in a day care setup that means there is no need of hospital admission for the patient. Laser glaucoma surgery can be done for both open angle glaucoma and angle closure glaucoma. Laser glaucoma surgeries are performed to lower the intraocular pressure ( IOL) and relieve the pressure on optic nerve. The length of time for which the IOL remains low depend on following factors.

  • Age of the patient
  • Type of glaucoma
  • Other medical conditions

Many patients who have undergone laser glaucoma surgery may still need the medications for lowing IOL but the dosage may be lowered.

Following Glaucoma laser surgery in India are performed at best eye surgery hospitals in India by top glaucoma surgeons.

Laser Trabeculoplasty in India :

It is a laser procedure and first line treatment for open angle glaucoma. Laser trabeculoplasty can be performed either by selective laser trabeculoplasty ( SLT ) or Argon laser trabeculoplasty ( ALT ) . In this procedure laser is targeted at drainage canals in eye and open it so the fluid can drain out smoothly and the intraocular pressure is reduced thereby preventing the optic nerve from damage. Procedure is done under local anesthesia. Patient may be prescribed some eye drops to reduce inflammation and swelling. Both SLT and ALT are similar but SLT is a newer type that uses gentler laser.

Laser peripheral Iridotomy or iridoplasty in India :

Laser peripheral iridotomy or iridoplasty is a standard first line treatment for angle closure glaucoma. In laser iridoplasty, the laser is targeted at the iris to make a hole in Iris to bring it back to it's original position. It opens up the drainage channel from posterior chamber into the anterior chamber by bypassing the pupillary block.

Laser cyclophotocoagulation glaucoma surgery in India :

Laser cyclophotocoagulation glaucoma surgery is done by using Cyclodiode laser. The laser targets the ciliary body. Ciliary body are the structures in eye that produces the aqueous humour. Objective of treatment is to reduce the production of aqueous humour. Treatment is done as a day care procedure under local anesthesia.

Best Glaucoma hospitals in India uses the latest MicroPulse cyclodiode device. It works in the same way as conventional cyclodiode laser device but uses short bursts of laser targeting the ciliary body. It allows the ciliary body to cool between pulses and reduce the damage to the surrounding tissue.

2. Incisional glaucoma Surgery in India :

Incisional Glaucoma surgery is done when the medications and glaucoma laser surgery fail to control the intraocular pressure and there is an evident risk of damage to optical nerve. Glaucoma incisional Surgery is also indicated for those glaucoma patients who have allergy to glaucoma medications, are not compliant to therapy or the disease continues to progress despite treatment. Advanced disease and other conditions are also indication for surgery. At best glaucoma hospitals , the top Glaucoma specialist doctors will discuss the best possible treatment options with the patients. While deciding about the Glaucoma incisional surgery and it's type, top glaucoma doctors consider the following factors:

  • The severity of the disease
  • how the patient has responded to glaucoma medications
  • Other underlying medical conditions and health issues with patient

The common incisional glaucoma surgeries done in India by the top glaucoma Surgeons at best glaucoma hospitals are listed below:

Incisional trabeculectomy glaucoma surgery in India:

The most commonly performed incisional glaucoma surgery in the world for treating glaucoma is trabeculectomy surgery. A small guarded fistula ( hole ) is made in the wall of the eyeball at the level of the junction of the cornea and the sclera. This guarded fistula allow the egress of the aqueous humor through a new alternative outflow way. Excessive aqueous humor is collected in a reservoir at the sub-Tenon/sub-conjunctival space that is called filtering bleb. Incisional trabeculectomy glaucoma surgery is generally done under the superior eyelid. The sutures control the amount of aqueous humor flow. From the bleb, the drained aqueous humor is then absorbed through the capillaries to the blood circulation and drained out of the eye. A small white coloured blister / scar can be seen in the upper part of the eye and is called a “bleb”. In most cases, the bleb remains covered by the superior eyelid and cannot be seen unless the eyelid is retracted.

A variation to trabeculectomy glaucoma surgery is to create a bleb with an Ex-Press shunt. The metal shunt does not affect MRI scans or airport security. Advantages of placing the shunt is that it hastens the vision recovery and less inflammation inside the eye after surgery.

Aqueous shunt or drainage implant glaucoma surgery in India :

An aqueous shunt or drainage implant glaucoma surgery involves putting a tube, shunt or other devices to create a new drainage channel. An implant is advised usually when other types of eye surgery fail to reduce the intraocular pressure. The aqueous shunt or implants consist of a silicon tube, which is connected to a draining plate. The tube is inserted in the eye and the drainage plate is sutured to sclera. The excessive aqueous humor is drained through or along the tube or shunt and collects around the plate at the back of the eye, where it is gradually absorbed by capillaries and the lymphatic system and transported out of the eye.

Minimally invasive incisional glaucoma surgery ( MIGS ) in India :

The minimally invasive incisional glaucoma surgery is an innovation or upgradation to conventional incisional glaucoma surgeries. With this new glaucoma surgery technique , surgery can be done earlier for glaucoma patients and more safely than conventional surgery.

Minimally invasive glaucoma surgery involves using smaller incisions and/or microscopic equipment, which have been designed to reduce risk associated with conventional trabeculectomy glaucoma surgery or the Aqueous shunt or drainage implant glaucoma surgery.

Glaucoma surgery for infants in India :

Two main types of surgery are done in India for infants with congenital glaucoma.

  • Goniotomy is used almost exclusively worldwide for infants with congenital glaucoma. In Goniotomy procedure, the doctor inserts a tiny blade through the cornea to cut the trabecular meshwork. This procedure allows the eye fluid to flow normally out of the eye.
  • Trabeculectomy is another glaucoma surgery used for infants with congenital glaucoma. This surgery involves making an incision in the outer portion of the eye and using a tiny probe to break through the trabecular meshwork. Aqueous humor is then able to drain out of the eye, keeping eye pressure in a normal range.

Deep brain stimulation (DBS) in India.

Deep brain stimulation (DBS) surgery in India - CMCS Health.

Deep brain stimulation ( DBS ) - The Process :

The concept of Deep brain stimulation ( DBS ) surgery evolved from the fact that the movement related disorders of Parkinson's disease and many other neurological disorders are caused by disorganized electrical signals in the areas of the brain that control movement. At CMCS Health , we along with our associated top Indian Neurosurgeons and intervention neurologists provide truly world class Deep brain stimulation ( DBS ) in India for both Indian and global patients.

How deep brain stimulation works :

The objective of Deep brain stimulation (DBS ) is to regulates abnormal electrical signaling patterns in the brain. Brain cells communicate with each other using electrical signals to control normal movement and other functions. When these signals become irregular and uncoordinated, it leads to motor symptoms. Deep brain stimulation (DBS ) interrupts the irregular signaling patterns so cells can communicate more smoothly and the symptoms are alleviated .

Deep brain Stimulation ( DBS ) process involves putting electrodes in either one or both sides of certain predetermined areas of brain ( either the Subthalamic nucleus or the Globus pallidus interna ) through a craniotomy surgery. These areas of the brain controls movements. The placement of electrodes in brain, during the Deep brain stimulation ( DBS) in India can be done in two ways.

  1. while the patient is fully awake and conscious during the implanting of electrodes, so that the Neuro-surgeon can ask questions and give you some simple tasks during the procedure. This is done to make sure that the electrodes are put correctly and at the desired site of brain.
  2. Procedure of implanting the electrodes in brain is done under general anesthesia with the help of imaging devices during the procedure for identification and putting the electrodes in the desired areas of brain.

The brain implanted electrodes are connected to a very small battery powered neurostimulator (electrical generator) placed under the person’s collarbone, similar to a cardiac pacemaker. This neurostimulator delivers continuous electrical pulses to the brain tissues through the electrodes. This neurostimulator also regulates and controls the amount of stimulation in deep brain stimulation (DBS ).

Deep brain stimulation ( DBS ) - Indications :

Deep brain stimulation ( DBS ) in India is the most common surgical procedure for Parkinson's disease. Deep brain stimulation ( DBS ) in India is taken up for correction of certain neurological disorders that includes:

  • Parkinson's disease
  • Essential tremors
  • Epilepsy
  • Dystonia
  • Obsessive compulsive disorders

Parkinson's disease :

Deep brain stimulation surgery(DBS) in India for Parkinson's disease.
Parkinson's disease - Deep brain stimulation (DBS) surgery in India | CMCS Health.

Parkinson's disease affects men almost 1.5 times more than women. Parkinson's disease is second most common neuro-degenerative disease of brain, after the Alzheimer. Parkinson's disease affects almost 1% of total population worldwide. It can occur even in younger people but the chances of younger people being affected with Parkinson's disease are very rare. As per the recent reports, currently more than 10 million people in the world are affected with Parkinson's disease.

Signs and symptoms of Parkinson's disease:

Parkinson's disease grows slowly, affects the nervous system and the body parts governed by the nerves. The symptoms start appearing gradually over years and may vary from person to person because of diversity of the disease. Symptoms of Parkinson's disease usually appear one side of the body first and then gradually affect the other side limbs too. The most common symptoms of Parkinson's disease are:

Symptoms of Parkinson's disease - Deep brain stimulation in India-CMCS Health.
Symptoms of Parkinson's disease- CMCS Health.
  • A rhythmic shaking of the limbs called Tremors. It usually starts in one side of the limbs first usually the fingers and hand. The tremor will be there when the arm is at rest but may decrease when some activity is performed by the arm.
  • Bradykinesia or slowed movements are also identified as symptoms for Parkinson's disease. Parkinson's disease makes simple task difficult and time consuming , even getting out of a chair becomes a big task. Patients stride while walking gets short and seems like dragging or shuffling of feet.
  • Parkinson's disease patient muscles also gets impacted and become rigid. The stiff muscles restrict range of movement and makes any movement painful.
  • Parkinson's disease patient's posture also changes from being upright to stooped. Patient may have balance problem and also develop an imbalanced gait.
  • Patients also looses involuntary movements like blinking, smiling or swinging of arms while walking.
  • Speech pattern changes occur in Parkinson's disease patients. Slurring sets in and initiation of a conservation becomes difficult.
  • A Parkinson's disease patient also have change in their handwriting and the usual writing strokes changes.

Deep Brain stimulation ( DBS ) for Parkinson's disease :

Parkinson's disease is a degenerative disease of brain usually occurring because of ageing but it can occur in people as young as 20 too. Parkinson's disease in young people is very rare. Although there is still no treatment for Parkinson's disease but new technological innovations have paved way for stopping or slowing the progression of the disease and improve quality of Parkinson's disease patients. At CMCS Health , we are associated with best Indian Neurology physicians , Neurology surgeons and intervention neurologists to offer effective control on symptoms and complications of Parkinson's disease patients. we offer cost effective deep brain stimulation (DBS) in India for Parkinson's Disease. Deep brain stimulation(DBS) surgery in India has benefitted many patients of Parkinson's disease from India and abroad.

DBS surgery for essential tremors:

Although deep brain stimulation ( DBS ) is approved for Parkinson's disease patients but patients of other movement disorders like Essential tremors also get benefitted with DBS.

Deep brain stimulation ( DBS ) for epilepsy:

Deep brain stimulation ( DBS ) in India is also used for those epilepsy patients who are 18 years or over in age and in whom seizures could not be controlled with anti epilepsy medications. DBS in epilepsy patients work by modulating brain cells and areas of the brain responsible for seizures through electrical stimulation. DBS is also used along with seizure medications for epilepsy patients.

Deep brain stimulation ( DBS ) for dystonia:

Earlier DBS was used only for severe cases of dystonia but in the last few years Deep brain stimulation surgery is used to treat greater varieties of dystonia patients.

DBS for obsessive compulsive disorders OCD :

Deep brain stimulation (DBS) is a newer treatment for major depression (MD) which is resistant to other treatments and obsessive-compulsive disorder (OCD). Obsessive-compulsive disorder (OCD) and major depression (MD) are common and disabling psychiatric disorders.Patients with MD or OCD may not respond well to pharmacological, psychotherapeutic, or somatic treatments. DBS was found to modulate dysfunctional neuronal networks involved in depression. It is documented in various studies that DBS is a promising alternative non ablative treatment for MD and OCD.

Selecting the right candidate for deep brain stimulation ( DBS ) :

Younger patients tolerate the DBS surgery better and may get benefitted more but there is no age restriction for the patients who can have deep brain stimulation (DBS) in India. Patients with significant dementia are not the right candidate foe deep brain stimulation ( DBS ) surgery.

It is evident from recently published data that deep brain stimulation (DBS) outcomes are best in improving quality of life and better control over motor function when it is performed before motor complications become disabling. So those Parkinson's disease patients in whom the motor dysfunction becomes evident should be considered for DBS before motor complications become disabling.

Parkinson's disease patients having mild cognitive disorders can be considered for deep brain stimulation ( DBS ) in India. But they need to be manged with medications for cognitive disorders . Patient with more severe forms or progressive changes may stand a risk of further deterioration triggered by the surgical process or DBS.

As with all elective brain surgeries, patient-specific medical conditions must be evaluated before deep brain stimulation ( DBS ) surgery.

  • Cardiac patients with a history of coronary vascular disease or history of angina should first be evaluated by a cardiologist before DBS.
  • Patients on antiplatelet agents for cardiovascular disease or anticoagulants for atrial fibrillation, pulmonary emboli, or deep venous thrombosis must temporarily stop taking these medications prior to deep brain stimulation surgery.
  • Patients who have co-morbid conditions such as hypertension and diabetes need proper control over hypertension and diabetes before DBS, to avoid intra-procedural and postoperative complications.

Why India for Deep brain stimulation (DBS ) surgery :

Top Indian hospitals offering neurological disease treatments are equipped with the latest , state of art technologies and neurology physicians , neuro-surgeons as well as intervention neurologists who are the masters in their craft.

  • The cutting edge technologies adopted by top neurology hospitals of India , combined with expertise and competency of Indian speciality doctors and high success rates have made India a preferred destination for global guests seeking Deep brain stimulation (DBS ) treatments. Cost of deep brain stimulation (DBS) in India is also quite reasonable as compared to other centers.
  • As DBS treatment may require a longer India stay , cost of rental apartments is a big consideration for patients seeking DBS treatment offshore. India offers many choices with readily available fully furnished guest houses and rented apartments in safe and secure environment that is close to the treating hospitals.
  • No waiting period for commencement of treatment.
  • Proper follow up of patient with treating Indian doctors once the patients reach back their respective countries.

Best in the world DBS devices available in India:

At CMCS Health , our associated deep brain stimulation treating hospital offers the choices among the best available DBS devices in the world. Although all the DBS devices are same in basic components and their function , but each device is designed and programmed in a different way to offer tailormade treatment for every individual patient . Some DBS devices offer the options of rechargeable batteries or electrodes that can deliver stimulation in novel ways or sense and record brain signals. The treating doctors will help in choosing the best suited DBS device for every individual patient by discussing about the suitability and pros and cons of every DBS device.

The best in the world DBS devices which are available in India are:

  • Infinity Deep brain stimulation system by Abbot.
  • Activa devices from Medtronic offers the choices of re-chargeable and non-rechargeable batteries.
  • Percept is the latest DBS device from Medtronic . It is first of its kind DBS device that can sense and record an individual’s unique brain signals.
  • Vercise from Boston scientific.

The rechargeable batteries may last up to 15 years (or more in some people) and non-rechargeable last, on average, about three to five years. The length of battery life depends on your individual settings. All current DBS devices also are safe for MRI, as long as certain safety conditions are met.

Cost of deep brain stimulation (DBS ) in India:

The cost of Deep brain stimulation ( DBS ) in India depends on the Indian hospital , expertise of the Indian doctor and the type and make of DBS device. Depending on these factors the average cost of DBS surgery in India:

Cost of investigations ( for ascertaining Suitability of DBS surgery , kind of device )800 - 1200 USD

It is at 20 minutes drive from New Delhi - Indira Gandhi international airport. Medanta is having a full fledged and dedicate neuro-sciences department with India's leading Neurologists and latest diagnostics facilities.

As per Medanta- The Medicity hospital - " Over the past few years, Brain Pacemaker surgery or Deep Brain Stimulation (DBS) surgery has emerged as an effective therapy in such cases. Herein, a neurosurgeon implants a device that sends electrical signals to specific brain areas, depending on the disease being treated. As the disease progresses, these signals can be re-programmed or modulated to accommodate changing symptoms.

The essence of a successful brain pacemaker surgery is its precision and accuracy. In this regard, the inculcation of cutting-edge technological advances in the conduction of a DBS stands to logic.
An ingenuous innovation, Directional DBS has emerged as a game-changer. Unlike a conventional DBS surgery, which only allows neuromodulation in one direction, the segmented electrodes (leads) of a Directional DBS system helps in steering the stimulation of the specific region of interest in all three dimensions, thus significantly enhancing the surgical focus and precision while minimizing inadvertent side effects.

Another promising technological advancement of DBS is a real-time Brain-Sensing biomarker-guided feedback analysis of the local field potentials by the brain pacemaker pulse generator itself which would then automatically release a closed-loop anticipatory stimulation response.

Recently, we implanted North India’s first Directional- and Brain Sensing-coupled DBS system in a 70-year-old patient suffering from incapacitating Parkinson’s disease for almost 15 years.
An interesting innovation nowadays being increasingly used in clinical practice of brain pacemaker surgery is patient-specific direct targeting of neural circuits, which is an emerging approach for refractory Parkinson’s disease patients as the next step forward."

ABO incompatible kidney transplants in India.

ABO incompatible Kidney transplants in India - CMCS health.

The need for ABO incompatible kidney transplants :

Living donor kidney transplants in India are usually done with a healthy blood-related (close relative) donor from the family of chronic renal failure (CRF) or end-stage renal disease (ESRD) patient. The preferred living kidney donor must be of the same blood group as that of the recipient or of the universally accepted “O” blood group. In the past, If an ESRD patient did not have a donor of the same blood group or of the “O” blood group, there was no hope of having a kidney transplant. The recent path-breaking advancement in Kidney transplantation, an ABO incompatible kidney transplant in India has ushered hope for such ESRD patients. Many of the Top Indian kidney transplant hospitals offer ABO-incompatible kidney transplants in India.

India is one of the most favored countries for patients seeking a living donor kidney transplant in a well-equipped and well-managed kidney transplant hospital. Most Indian kidney transplant hospitals offer hassle-free and truly global-class kidney transplant surgery with success rates that are at par with the best Kidney transplant hospitals in the world. The reasonable cost of kidney transplant surgery with almost no waiting period has made Indian Kidney transplant hospitals a favorite place for kidney transplant needing foreigners.

The first successful Kidney transplant took place in India in 1971 at Christian medical college Vellore. Since then many Indian hospitals have performed successful kidney transplants year after year for Indian and foreign patients.

India's first successful ABO kidney transplant took place in 2011 at Kamineni Super Specialty Hospital in Hyderabad city between a father son duo. Father, who was a living kidney donor had Blood group "A+" where as his kidney recipient son had a blood group " O+".

Now many Top hospitals in India are performing ABO incompatible kidney transplants in India for both Indian and foreign patients seeking kidney transplant in India.

ABO incompatible kidney transplants are universally accepted for those young ESRD patients who does not have a blood group matching donor.

ABO incompatible Kidney transplant involves desensitization of the recipient before the kidney transplant surgery to overcome the ABO incompatible antibody induced kidney graft rejection. For a successful and long lasting ABO-incompatible kidney transplantation, temporary elimination of the anti-ABO antibodies from the recipient's serum is an absolute must.

Desensitizing an ESRD patient before ABO incompatible kidney transplants :

The process of desensitizing an ESRD patient before ABO incompatible kidney transplants in India is aimed to reduce Anti A/B antibody levels to achieve titers between 1:8 and 1:32.

Desensitizing an ESRD patient before an ABO incompatible kidney transplant involves following key stages:

B- Cell depletion before ABO incompatible Kidney transplants :

Monoclonal antibody therapy with an anti-CD20 drug rituximab have eliminated the earlier practice of splenectomy. In addition therapy with rituximab, prior to ABO incompatible kidney transplant deplete B-cells and plasma cells by complementing dependent cytotoxicity (CDC) , and antibody dependent cellular cytotoxicity.

Apheresis before ABO incompatible kidney transplants:

The therapeutic apheresis is a cornerstone for ABO blood group incompatible kidney transplants.Therapeutic apheresis consists of separating and selectively removing the pathogenic material from the patient by an extracorporeal aphaeresis system. The two most common and universally accepted methods used for apheresis are:

1. Double-filtration plasmapheresis (DFPP):

Double filtration plasmapheresis is done by using two plasma filters. The second filter allows smaller molecules to return back to the patient. DFPP helps in avoiding many complications associated with the ordinary therapeutic plasma exchange. DFPP uses centrifugation or a plasma separator, and double-filtration plasmapheresis (DFPP).

Double filtration plasmapheresis technique aims at selectively removing the immunoglobulin fraction from the serum and minimizing the volume of substitution fluid required. Human plasma albumin is used in this technique is used as replacement fluid.

Immunosuppression is also started in pre transplant ABO incompatible recipients 7 days before the transplant surgery as a pre transplant conditioning. Immunosuppression drugs generally used in India for ABO incompatible kidney transplants are tacrolimus , mycophenolate mofetil and methyl prednisolone .

2. Immunoadsorption (IA):

Immunoadsorption is a more effective with least side effects in removing anti A/B antibodies. Immunoadsorption (latest the apheresis method) , combined with hemodialysis is used to decrease donor-specific alloantibodies in ABO incompatible kidney transplant recipients before kidney transplant. Different types of columns are used in the process.

  • A Therasorb column contains polyclonal sheep anti-human IgG antibodies .
  • Immobilized staphylococcal protein A using columns.
  • Immobilized antigens and synthetic epitopes using columns

The major advantage of immunoadsorption over other apheresis techniques is that, it minimizes the need for replacement of factors such as albumin and plasma.

Studies demonstrate that Desensitization of the human-leukocyte antigen using this immunoadsorption procedure effectively reduced or eliminated donor-specific alloantibodies in 71% of patients undergoing ABO incompatible kidney transplants.

Rise in numbers of ABO incompatible kidney transplants:

Earlier the Living donor Kidney transplants were done with only donors having same blood group as that of the recipient. This was restricting the pool of available living kidney donor for an ESRD or CRF patient who were not having a matching blood group donor. Many ESRD patients who had a willing and healthy kidney donor from the family, could not have a kidney transplant because of blood groups not matching.

With the newer Plasma apheresis techniques and protocols that can successfully bring down the A/B blood group antibodies in ABO incompatible ESRD patients , It is possible now to have a successful Kidney transplant with an otherwise fit kidney donor having a different blood group.

Recent studies have shown that with proper pre transplant treatments for ABO incompatibility , patients undergoing the ABO incompatible kidney transplants have almost the same success rate as that of same blood group recipient and donor. Even the longevity of newly grafted kidney was found to be almost the same.

Which ESRD patients are ideal for an ABO incompatible kidney transplants :

Any ESRD patient who have a fit blood related living donor but with the different blood group can be considered for ABO incompatible kidney transplants. The feasibility of an ABO incompatible kidney transplant depends on the quantity of donor specific ABO incompatible antibodies present in recipient in the beginning , and how much the desensitization programs will help in reducing the ABO incompatibility induced antibodies.

If even after the desensitization programs the ABO incompatibility induced donor specific antibodies does not fall below the specified levels , doctors may not recommend an ABO incompatible kidney transplant.

Other parameters remains same as that in a same blood group matching donors.

An ESRD patient can not have a kidney transplant if he/she is having following medical conditions:

  • An ESRD patient who is too weak and frail to cope up with surgery and post transplant management protocols.
  • If the ESRD patient is known to have serious cardiovascular deficiency or has a recent heart failure or stroke episode.
  • An ESRD patient who had a recent serious infection or cancer.

Precautions posts a successful ABO incompatible kidney transplant :

The Nephrology doctors team monitor the progress and functioning of newly grafted kidney in a successfully ABO incompatible kidney transplanted recipient.

The first two weeks are crucial after a successful ABO incompatible kidney transplant surgery for the acceptance of newly grafted kidney. After the first two weeks the chances of kidney graft rejection due to A/B blood group incompatibility induced antibodies start diminishing significantly.

Doctors recommend frequent blood tests to monitor the newly grafted kidney's function.

Post successful ABO incompatible kidney transplant the precaution remains same as that of a blood group matching transplants. To know more about precautions for a newly grafted kidney last your life time ,read our exhaustive blog on the subject.

Cost of ABO incompatible kidney transplant in India:

There may be variations in ABO incompatible kidney transplant cost . This variation could be because of the process of B-cell depletion , plasma apheresis or immunoadsorption techniques adopted for every individual patient.

Cost of pre transplant investigations800-1500 USD
Cost of Desensitization for reducing A/B blood group incompatibility induced antibodies (Conditioning)6000-10000 USD
Cost of ABO incompatible kidney transplant 13500-15500 USD
Cost of ABO incompatible kidney transplant in India .

Why choose India for an ABO incompatible kidney transplant:

Many ESRD patients who do not have a blood group matching donor but an otherwise fit and blood related family member willing to donate a kidney for the patient are preferring India for ABO incompatible kidney transplants. The reasons for choosing India for ABO incompatible kidney transplants are as follows:

Best Brain AVM treatment in India.

A Brain AVM if not treated in time, may cause a brain hemorrhage or a brain stroke and can be fatal at times. At CMCS Health , we provide Best Brain AVM treatment in India with top Neurology- brain surgeons and Intervention Neurologists at best brain surgery hospitals in India.

Arteriovenous malformation ( AVM) of Brain

Arteriovenous malformation ( AVM) is an abnormal connection between the arteries and veins. Both arteries and veins are part of the circulatory system. Arteries are the blood vessels in the body that carry oxygenated blood from the heart to all the cells in body. Arteries distribute oxygen , nutrients and hormones to all types of cells throughout the body. Whereas veins carry the deoxygenated blood from all the cells in body to heart. An arteriovenous malformation(AVM) usually occur in brain or spine but it can occur anywhere in body. An arteriovenous malformation (AVM) in brain is abnormal, snarled tangles of blood vessels that cause multiple irregular connections between arteries and veins in brain.

Why early Brain AVM treatment is needed :

The heart sends oxygenated blood to the brain through arteries. The arteries pass this blood through a series of progressively smaller networks of blood vessels, ending with the smallest blood vessels (capillaries). The capillaries slowly deliver oxygen through their thin, porous walls to the surrounding brain tissue. The deoxygenated blood passes into small blood vessels and then into larger veins that return the blood to the heart and lungs to get more oxygen. The multiple irregular connections between brain arteries and veins bypass the process of blood flow through these small blood capillaries and vessels. When the small capillaries' and vessels in brain are devoid of oxygenated blood and nutrients. The arteries and veins as well as the blood capillaries in brain can rupture because of the arteriovenous malformation( AVM) and cause bleeding in brain ( hemorrhage ) or a brain stroke.

We provide best brain AVM treatment in India at Top neuro-surgery or brain surgery hospital in India. Get a quotation about Best Brain AVM treatment cost in India with top brain surgeons in India.

Arteriovenous malformation ( AVM) Brain occurrence:

Brain AVMs are usually present in a child at the time of birth ( congenital ) but it can develop later too. A Brain AVM may be found soon after birth of the child or much later in life when the child grows into an adult, depending on its size and location. As a patient's body grows, the AVM grows too and become evident.

Categories of Arteriovenous malformation (AVM) :

AVMs are often categorized using a scale called the Schöbinger staging system.

  • Stage I (quiescence): The AVM is "quiet." The skin on top of the AVM may be warm and pink or red.
  • Stage II (expansion): The AVM gets larger. A pulse can be felt or heard in the AVM.
  • Stage III (destruction): The AVM causes pain, bleeding or ulcers.
  • Stage IV (decompensation): Heart failure occurs.

Complications of Brain AVM:

An untreated Brain AVM may cause serious complications. We not only provide the accurate diagnosis of Brain AVM but also the best Brain AVM treatment in India with Top brain surgery doctors in India.

  • An increased pressure against the walls of Brain AVM associated arteries and veins make them weak and thin, this may make them prone to rupturing. A rupture in Brain AVMs may cause hemorrhage or bleeding in brain.
  • Reduced oxygen to brain tissues due to deprivation of oxygenated blood supply make them weak or even gradually die. This will cause in stroke-like symptoms, such as difficulty speaking, weakness, numbness, vision loss or severe unsteadiness.
  • As The Brain AVM grow in size because of increased blood flow to them, they may compress brain tissues and push them towards the skull causing irreversible brain damage.

Do not delay the Brain AVM treatment. CMCS health connects you with top brain surgeon in India at best brain surgery - Neuro surgery hospital for a cost effective Brain AVM treatment in India.

Signs and symptoms of Brain AVM:

Many of Brain AVM patients does not have any early signs and symptoms. Many Brain-AVM patients shows signs only when AVM ruptures, resulting in hemorrhage. In about half of all brain AVMs, hemorrhage is the first sign.

Some Of the Brain AVM patients may have following signs and symptoms.

  • Headache or pain in one area of head.
  • Numbness or muscles weakness in any part of the body.
  • Seizures.

Other Brain AVM patients may experience more severe signs , depending on the location of Brain AVM.

  • Severe headache
  • Weakness, numbness or paralysis
  • Vision loss
  • Difficulty speaking
  • Confusion or inability to understand others
  • Severe unsteadiness

If you observe the above symptoms , send your query to us. We will connect you with Top brain - Neurosurgeon of India to help you in getting opinion anout treatment course and cost.

Risk Factors for Brain AVM:

  • Being a male increases the risk of Brain AVM.
  • Women during pregnancy may have a ruptured brain AVM.
  • Certain genetic conditions such as hereditary hemorrhagic telangiectasia (HHT) may increase the risk factor for Brain AVM.

Diagnosis of Brain AVM in India:

Most of the JCI accredited best brain surgery hospital in India have Latest diagnostic tests for accurate diagnosis of Brain AVM. The commonly conducted diagnostic procedures for brain AVM includes:

Brain Ultrasound for diagnosis of Brain AVM:

The first test that a clinician may recommend on suspicion of a Brain AVM is an ultrasound. An Ultrasound captures pictures of the blood vessels and tissues under the skin by using soundwaves. An ultrasound of brain also marks the speed of blood flow, which helps the clinician in diagnosing a Brain AVM.

An ultrasound is a preferred diagnostic tool for brain AVM diagnosis in children as it is completely pain free and does not require anesthesia.

Computerized tomography (CT scan) of brain for diagnosing Brain AVM:

CT scan of the brain create a detailed cross-sectional image of the brain by using a cluster of X-rays. A CT- scan of the brain will be able to highlight if the Brain AVM is pressing against a skull bone.

Magnetic resonance imaging (MRI) for Brain AVM:

MRI of Brain is more efficient than brain CT Scans in showing more subtle changes in brain tissues due to Brain AVM. MRI of brain provides more detailed pictures of the size and location of an AVM inside the brain. An MRI of the brain will also highlight the critical structures near the Brain AVM like a nerve that may be affected during the Brain AVM treatment.

Digital subtraction Angiography (DSA) for diagnosis of Brain AVM:

A Brain DSA delivers a very detailed image of the blood vessels in the brain to detect a problem with blood flow. Procedure is done under the general anesthesia. The procedure involves inserting a catheter (a small, thin tube) into an artery in the leg and passing it up to the blood vessels in the brain. A contrast dye is injected through the catheter and X-ray images are taken of the blood vessels. A Brain DSA test diagnose and "map" the blood vessels in an AVM and are also used during treatment of an AVM.

Best Brain AVM treatment in India:

We offer complete and comprehensive Best Brain AVM treatment in India with best Indian intervention neurologists and top brain surgeons in India. The factors that influence the treatment of Brain AVM are:

  • Size and location of Brain AVM.
  • Medical complaints of patient due to Brain AVM
  • Age of the Brain AVM patient.
  • Overall health of the Brain AVM patient.

The Goal of Brain AVM treatment is to prevent brain hemorrhage and control seizures and other neurological conditions.

Craniotomy - microsurgery | Best Brain AVM treatment in India :

Microsurgery is considered as a gold standard for AVMs treatment. A small nidus (up to 3 cm) can be completely removed in more than 94% of patients, with a good overall outcome in more than 90% cases .

If the Brain AVM has bled and is easily approachable, without harming any brain tissues or structures, then the neurosurgeon remove part of the skull temporarily ( Craniotomy ) to gain access to Brain AVM.

Top Neuro-Surgeons in India will use a high magnifying microscope to seal off the Brain AVM with special metallic clips and carefully removes the AVM from the surrounding brain tissues. After successful removal of Brain AVM the surgeon will reattach the skull bone and close the incision in the scalp.

Microsurgery for Brain AVM is done only after the neuro-surgeon assess that Brain AVM can be safely removed without the risk of further hemorrhage or seizures . For the Brain AVMs that are deeply rooted in brain or which are not easily accessible for surgical resection or when the treating top brain surgery doctor in India feels that surgical removal is too risky , other treatment options are discussed with the patient for best Brain-AVM treatments in India.

Use of latest technologies like neuro-navigation or indocyanine- green video-angiography by the best Brain surgery hospital in India have improved the results of microsurgery for brain AVMs in India considerably.

Endovascular embolization - Best Brain AVM treatment in India :

Endovascular embolization in India, using coils or Onyx, may be used for nidus reduction and sometimes for complete obliteration of Brain AVMs.

In endovascular embolization a catheter is inserted into an artery in the leg or wrist and is threaded through blood vessels to the brain using X-ray imaging. The treating doctor has already identified an artery that feeds the Brain AVM and injects an embolizing agent such as small particles, a glue-like substance, micro coils or other materials to block the identified artery feeding the brain AVM and reduce blood flow into the AVM.

Endovascular embolization is less invasive as compared to microsurgery for resection of Brain AVM.

However endovascular embolization does not offer a cure for Brain AVM . Over a period of time the Brain AVM may regrow.

At times Top brain surgery doctors in India treating Brain AVM recommend endovascular embolization before craniotomy for microsurgery of Brain-AVM, to reduce the size of Brain AVM and also the risk of bleeding during the surgery.

You can enquire about Endovascular embolization cost in India for brain AVM treatment from the Top brain surgeons in India by sending all your Brain scans , MRIs of brain to us.

Sclerotherapy Treatment - Best Brain AVM treatment in India :

Like endovascular embolization sclerotherapy is also used for reducing the size of Brain AVM. In sclerotherapy, a fluid (sclerosant ) is injected into the AVM to destroy the vessels and cause scars to form. These scarring will cause less or no blood flow through the AVM.

Sclerotherapy is also used to treat other vascular malformations, such as venous malformations and lymphatic malformations.

Both endovascular embolization and sclerotherapy does not cure the Brain-AVM but reduces the size of Brain AVMs and also relieve the patient from symptoms.

CyberKnife or Gamma Knife ( stereotactic radiosurgery) treatment | Best Brain AVMs treatment in India :

Stereotactic radiosurgery is preferred choice for those Brain AVM patients where the Brain AVMs are deep rooted in brain tissues and surgery is not possible because of non-accessibility or the major risks outweigh the benefit of surgery. We are associated with Best CyberKnife treatment specialists and Top Neuro-surgeons of India for best Cyberknife treatment for brain AVM.

Stereotactic radiosurgery is not surgery in the traditional sense as there is no incision or cut. Instead, stereotactic radiosurgery uses 3D imaging to target high doses of radiation beams to the affected area with minimal impact on the surrounding healthy tissue.

Stereotactic radiosurgery of the brain and spine is typically completed in a single session.

Best Hospitals for meningiomas treatments in India.

Best Meningioma hospitals in India.

Meningioma - Best meningioma hospitals in India..

Meningiomas are slow growing tumors occurring in membranes of brain and spinal cord ( meninges). Meningioma are more common in African Americans and women. If not treated in time , meningiomas can grow big and cause serious neurological disorders. Untreated meningiomas may be fatal too. At CMCS Health , we offer cost effective meningioma treatments in India with best Indian neurology specialist doctors at best meningioma hospitals in India.

Meningiomas start in the meninges which are the membrane surrounding the brain and spinal cord. Meningiomas are generally slow-growing tumors of the brain and spine. As meningiomas grow slowly it is difficult to diagnose them in the early stage. These benign tumors are discovered when they become very large and start compressing the surrounding tissues. If left untreated certain meningiomas because of their size and location can cause severe neurological disorders and may even be fatal. Meningiomas are generally slow-growing tumors but in certain diagnosed cases they were found to be aggressive too. Most patients develop a single meningioma; however, some patients may develop several tumors growing simultaneously in other locations of the brain or spinal cord.

Signs and symptoms of meningioma:

Symptoms of meningioma .

Meningiomas are slow-growing tumors, symptoms are not very evident in the beginning. However the most common signs and symptoms of meningiomas could be :

  • Changes in vision, double or blurry vision
  • Persistent headaches that are worse in morning time
  • Gradual or sudden hearing loss or tinnitus
  • Gradual or sudden memory loss
  • Confusion or language problems
  • New seizures
  • Weakness in limbs ( hands and legs )
  • Sudden or gradual loss of smell

Meningioma symptoms may appear because of size and location of tumors.

Risk factors associated with meningioma:

Risk factors associated with meningioma .

Although the exact cause of meningioma is still not known but following factors are identified as a possible cause of development of meningioma. The risk of meningioma increases with age with a dramatic increase after 65 years. It is observed in studies related to meningiomas that African Americans are at higher risk of getting meningioma than other ethnic groups. Other risk factors associated with meningiomas are:

  • An earlier exposure to Radiation.
  • Being a woman increases the risk of meningiomas as female hormones estrogen , progesterone and usage of oral contraceptive pills and hormone replacement therapies are found to be associated with an increased risk of developing meningiomas.
  • An inherited medical condition ( genetic disorder ) Neurofibromatosis- Type 2 increases the risk of meningioma.
  • Obesity or an increased BMI ( body mass index ) increases the risk of meningioma.

Tests done at best meningioma hospitals in India for Diagnosis of Meningioma:

Meningioma diagnosis - CMCS Health.

Being slow-growing tumors and mixed symptoms with other medical conditions, the diagnosis of meningiomas is not easy. If a patient complains of slowly increasing signs of mental dysfunction, new seizures, or persistent headaches, or if there is evidence of pressure inside the skull, doctors may suspect a meningioma and advise the related investigations to confirm the diagnosis. At CMCS Health , we are associated with best meningioma hospitals in India for accurate and precise diagnosis of meningioma. Doctors may evaluate the patient with Imaging tests and a biopsy to confirm the diagnosis.

To diagnose a meningioma, Our expert Indian Neurologists at best Meningioma hospitals in India conduct a thorough neurological exam followed by an imaging test with contrast dye :

Computerized Tomography ( CT ) Scans for meningioma :

CT scans uses X rays to take cross sectional images of full brain and spine for diagnosis of meningiomas. At times an Iodine based dye ( contrast media ) to make the images more clear.

Magnetic resonance Imaging ( MRI ) for meningioma :

In MRIs magnetic fields and radio waves are used to create cross sectional images of structures inside the brain and spine. MRI scans provide more detailed images of brain and spine for accurate diagnosis of meningiomas.

Intra-operative MRI for meningioma treatments :

Intraoperative Magnetic Resonance imaging ( iMRI) uses magnetic fields and radio waves for creating accurate images of brain or spine when the neurosurgeon is performing the surgery for removal of tumors. Another advantage or iMRI is that it helps your neuro-surgeon in complete excision of meningioma tumors of brain and spine without harming critical tissues nearby the tumors.

Tumor biopsy:

Brain or spine tumor biopsy is a more confirmatory test for diagnosis of meningioma. Biopsy of brain or spine for meningioma also helps the clinician in deciding about the best treatment for meningioma.

Grades of meningioma :

Majority of brain tumors are categorized in 4 grades but meningiomas are categorized in 3 grades.

Grade -1 Meningioma:

Tumor cells looks least abnormal . Most benign operable meningiomas can be successfully treated with surgery or if they are inoperable , Radiation ( CyberKnife) can treat them effectively.

Grade - 2 Meningiomas:

Tumor cells look more abnormal. These meningiomas can grow and invade the surrounding tissues.

Grade 3 Meningiomas:

In Grade 3 meningiomas , tumor cells looked most abnormal. These meningiomas are aggressive type and can grow rapidly, also the recurrence rates are high with grade 3 meningiomas.

Treatments of Meningioma In India:

Meningioma Treatment in India.

Treatments for a meningioma depend on symptoms, type, location, patient’s health, and age. Treatments for Meningiomas may include:

Surgery for treatment of Meningiomas in India :

Meningiomas are generally benign tumors with well defined boundaries. Doctors treating meningioma may decide about microsurgery for removing the whole tumor along with the fibers that attach it to the coverings of the brain and bone. Our Meningioma specialist doctors at the best meningioma hospitals in India opens the skull through a craniotomy to enable full access to the meningioma. However sometimes it is not possible to remove the meningioma completely because of it's proximity to critical nerves ,veins and tissues. Our expert neuro-surgeons treating meningioma perform preoperative embolization of the tumor to ensure safety during the surgical procedure.

For patients with whom total removal of the tumor carries significant risk of morbidity (any side effect that can cause decreased quality of life), it may be better to leave some of the tumor in place. Such patients are advised to have periodic CT scans or MRIs for keeping a track on , if the leftover tumor has started increasing in size.

Post surgery care for Meningioma patients :

If post surgery observation in meningioma patients shows no visible tumors or any increase in size of leftover tumors, no further treatment will be needed.

Cyberknife ( Radiation therapy ) treatment in India for Meningiomas:

The CyberKnife uses highly focused and precisely targeted radiation beams, from different angles that destroy the tumor while sparing surrounding healthy tissue. At CMCS health , we are associated with best Cyberknife treatment specialists in India at Best meningioma hospitals in India.

Cyberknife treatment for meningioma - CMCS health.

A major advantage to CyberKnife is that it’s non-invasive and doesn’t require a metal frame be screwed into the patient’s skull to perform treatment like with the Gamma Knife.

Meningioma symptoms like headaches, double vision, seizures, or other symptoms will improve after CyberKnife treatment, though improvements typically take several months or longer in some cases.

Chemotherapy for meningioma treatments in India:

Although chemotherapy is rarely used for meningioma patients. But in some patients of meningioma where surgery was not able to excise the whole tumor or the tumor is found to be atypical or malignant , doctors may use chemotherapy to reduce the risk of recurrence .

At CMCS health , we offer complete and comprehensive treatment for both brain and spine meningiomas.

What is CyberKnife Treatment for tumors:

CyberKnife is a modern radiation technique that consists of a lightweight linear accelerator that is mounted on a robotic arm. An image-guided system tracks the real-time location of the tumor in the patient’s body and the linear accelerator targets high doses of radiation beams at the tumor from different angles. With Cyberknife the clinician has better maneuverability to deliver high doses of radiation to the tumor with great accuracy and precision, and with negligible harm to surrounding tissues.

What are advantages of CyberKnife SRS treatments:

The biggest fear in the minds of those diagnosed with cancer was that of immediate surgery. True! Surgery remains the gold standard for many early-stage, non-metastatic cancers. The advent of the latest modern technologies has provided many effective treatment alternatives for surgery. CyberKnife robotic radio-static surgery system has emerged as a very useful alternative to surgery for many tumor patients.

Most importantly, one can trust that Cyber-Knife will work, as it is clinically proven effective in treating tumors that are complex and inoperable.
  • CyberKnife treatment offers better patient compliance. CyberKnife is a highly sophisticated radiation delivery system because of it's real-time imaging system and a flexible robotic arm . CyberKnife SRS system can position itself to the exact location and angle to deliver high dose radiation beams at different angles to the tumor. Because of pin pointed precision and accuracy it targets only the tumor cells and avoids damaging the surrounding healthy tissue, creating less potential side effects than traditional radiation. Moreover CyberKnife is made to move with patient's body and the tumor, thus there is no need for stabilizing frames pinned to patient's head, body casts or breathing-limitation devices that are needed to limit patient's movement.

Top breast cancer surgeons in India offers the safest breast cancer surgery in India at best breast cancer hospitals in India.

Some breast cancer patients may experience following complications post a successful breast cancer surgery:

  • Formation of a blood clot around the surgery wound. This may cause hardness, pain and swelling around the surgical wound. This hematoma usually subsides on it's own but in some patients it may take a few months. Your breast cancer surgeon may advise draining if if the swelling is bothering you. A blood clot may also form in the legs post successful breast cancer surgery . Rarely these blood clots may travel to lungs and may cause a serious medical condition. Your breast cancer surgeon will advise you to start walking and do leg exercises soon after the breast cancer surgery to prevent formation of blood clot in your legs.
  • A small bleeding from surgical wound that soaks the wound dressing is ok , post successful breast cancer surgery but if you notice persistent and moderate bleeding from the surgical wound , bring it to notice of your breast cancer surgeon, he will advise accordingly.
  • Surgical wound infection is also a possibility post a successful breast cancer surgery. If you feel pain at the wound site , observe any unusual discharge from surgical wound , the skin around the wound seems red , swollen and hot, or run a temperature , there is a possibility of surgical wound infection. Your doctor may advise a blood culture for knowing the cause of infection and will prescribe antibiotics for wound infection.
  • Seroma ( Edema near the surgery wound ) is accumulation of fluid near the surgery wound ,causing swelling. If seroma is painful , your treating doctor will drain ( tapping ) this accumulated fluid with the help of a syringe and needle to relieve you of any pain because of seroma.
  • Feeling of numbness, tingling or a shooting pain in your armpit, upper arm, shoulder or chest wall. This is due to damage to the nerves during surgery. The nerves usually repair themselves soon after the breast cancer surgery , but in some breast cancer patients it may take many weeks or months. Your treating doctor will prescribe nerve pain relieving medicines to give you relief from pain.
  • Stiffness of shoulder may occur in some patients who have undergone a breast cancer surgery and axillary lymph node removal. Your treating doctor will recommend you to a physiotherapist for specially designed physiotherapy and exercise regimen to restore mobility and pain relief from shoulder joint stiffness.
  • Lymphedema may occur in some breast cancer patients post surgery and lymph node removal. Small swelling in arm and hand is ok and it subsides quickly after surgery . If this swelling does not settle soon after the surgery and it persists along with pain and tenderness in hand and arm , consult your doctor. This is swelling caused by lymph fluid that can't drain away. It can happen any time after surgery. Once you have lymphoedema it can’t be cured but early treatment can help to control it. Your nurse will talk to you about ways of preventing lymphoedema. Read our exhaustive blog on Lymphoedema to know more about it.
  • Scar tissue in the armpit (cording) is scarring of tissue in the armpit after lymph node removal. The connective tissues in the armpit get inflamed, which forms one or more tight bands. Cording is harmless but can be painful and can limit your arm movement. Your treating doctor will recommend you to a physiotherapist for specially designed physiotherapy and exercise regimen to ease the pain.

Recovering after a breast cancer surgery:

Breast cancer surgery wound generally heals in 4 to 8 weeks . Your breast cancer treating doctors may recommend Radiation therapy or even chemotherapy post breast cancer surgery to offer comprehensive breast cancer treatment in India and reducing the chances of breast cancer coming back.

Chemotherapy or radiation therapy post breast cancer surgery is done when the breast cancer surgery wound heals completely.

Dos and Do nots after a breast cancer surgery:

  • Pain management is important in breast cancer surgery as in any other surgery. The intensity of pain and discomfort may vary from one patient to other. It is advisable to take pain medication prescribed by your doctor before the pain becomes severe. Healing and recovery is faster with effective main management. Applying an icepack to the armpit and lymph dissection area decreases the swelling and discomfort. A small pillow put in armpit may also help in decreasing the discomfort.
  • Breast cancer surgery wound care is important for not getting it infected. Your treating doctors team will guide you about wound care at the time of your discharge from hospital.
  • You will be discharged from hospital post your breast cancer surgery with a drain in place. This drain consists of a plastic tube from under the skin to outside your body with a bulb attached to it. You will be asked to empty the drain two to three times per day or when the bulb is full. You will be asked to write down the amount drained per day on a sheet of paper and once it comes down to a certain level your team of doctors will remove the drain and cover the opening with a dressing for healing.
  • If axillary lymph nodes are also removed in your breast cancer surgery, your team of doctors will advise you about arm care on that side. You are advised not to lift heavy objects with that arm and you may be asked to wear a sleeve on that side arm for some time. You will be told to do some physiotherapy exercises for your arm once the wound heals. You will also be advised to not get any injection or needle insertion on the affected side arm ( administering of any medication including antibiotics or collection of blood for pathological tests). Your doctors team may also advise that taking blood pressure from this arm should also be avoided.
  • You will also be educated about the risk of lymphedema post the breast cancer surgery.

After completing her remaining chemotherapy cycles in Afghanistan , she visited us again and the imaging tests confirmed complete response , even the axillary lymph node was dissolved completely. She had her Modified radical mastectomy performed by leading breast cancer surgeon in India , Dr. Deepak Jha at Artemis hospital Gurugram , India.

After complete healing of her breast cancer surgery wound , she was advised to have radiation therapy for reducing the chances of recurrence.

After her radiation therapy sessions and consultation with her breast cancer treating team of doctors , she left for Afghanistan .

The first thoracic CT scan after six months of her radiotherapy showed no growth of any new tumor. It was encouraging.

CT scan of thoracic region and bone scan after one year of treatment had also shown no disease progression. She visited India after 2 years of successful breast cancer treatment , PET CT reconfirmed her status of being disease free. With every passing year the chances of recurrence of disease goes down significantly. Today after almost 5 years her disease is in complete remission. Types of breast cancer :

Breast cancers are classified, depending on the types of cells involved.

Ductal Carcinoma in situ - DCIS ( Non invasive ):

Ductal carcinoma in situ is abnormal cells in the milk duct of the breast. They appear as a cluster of irregularly shaped and sized cells which are calcified. Ductal cell carcinoma usually does not have signs and symptoms but is discovered on a routine mammogram examination of the breast or for investigation of a breast lump. Ductal carcinoma in situ is also called noninvasive ductal carcinoma as they are limited to milk ducts only and have not infiltrated to surrounding fatty tissues of the breast. Ductal carcinoma in situ are the precursor form of breast cancer and a treatment is considered by the breast cancer specialist only if a lump appears in the breast tissues, noticing an area of puckered or otherwise unusual skin, a thickened region under the skin, or unusual nipple discharge.

Depending on age and considering the risk factors breast cancer specialists may recommend breast-conserving surgery or a surgery to remove all of the breast tissue combined with radiation therapy.

Invasive Ductal cell carcinoma ( IDC):

Invasive ductal cell carcinoma is the most common of all breast cancers and accounts for almost 75 to 80 % of breast cancers. Invasive ductal cell carcinoma starts in the milk ducts of breast but spreads to fatty tissues of the breast. It can also spread to auxiliary lymph nodes and other distant tissues and organs through the lymphatic system and blood stream.

When it is spread out of the breast tissues , it is called metastatic invasive ductal cell breast cancer.

Lobular carcinoma in situ-LCIS ( Non invasive):

Lobular carcinoma of breast is forming of abnormal cells in the lobules of breast or milk glands. Lobular carcinoma in situ is not considered a breast cancer but a women diagnosed with LCIS is considered at higher risk of developing breast cancer. If a women is diagnosed with LCIS , doctors may recommend Regular and periodic screening of breast including mammogram and will also discuss treatment plans so that LCIS does not transform into invasive carcinoma.

Invasive Lobular carcinoma- ILC:

Invasive lobular carcinoma ( ILC ) are the second most common breast cancer in women. These cancers start in milk producing ducts or lobules of breast and infiltrate or invade the other tissues. Most of the invasive lobular carcinomas are estrogen receptor positive ( ER+) and HER2 negative.

ILC are slowing growing breast cancers and does not respond well to chemotherapy , hormonal therapy is key to treatment of invasive lobular carcinoma of breast.

Inflammatory breast cancer ( IBC ) :

Inflammatory breast cancers are very rare breast cancer types. They account for only 1 to 5 % of all breast cancers. Inflammatory breast cancers are invasive ductal carcinomas, which are developed from cells that line the milk ducts of the breast and then spread beyond the ducts. They are the most aggressive of all breast cancers and require prompt treatment. Inflammatory breast cancers are majorly hormone-negative and thus hormone treatments do not work with IBC.

Inflammatory breast cancers are more common in African American women as compared to fair-skinned women. IBC are more common in obese women than the women who are on leaner side.

Angiosarcoma of breast:

Angiosarcoma of the breast are very rare cancer and 1 out of 100 women may have angiosarcoma of the breast. Angiosarcoma of the breast usually forms in the lining ( walls) of lymphatic vessels or blood vessels. Angiosarcoma of the breast grows quickly and is difficult to treat. Angiosarcoma develops generally in people older than 70 years of age and sometimes previous radiation therapy to the breast may be one of the causes.

Paget's disease of the breast:

Paget's disease of breast is also a very rare cancer. Paget's disease of breast starts in the skin of nipple or areola.

The prognosis for Paget's disease breast depends on several factors, including the presence or absence of invasive cancer and whether or not it has spread to nearby lymph nodes.

Other classification of breast cancers:

Breast cancer can be further subdivided on expression of certain receptors. It helps in determining the treatment course also. It is usually done once the biopsy reports of breast tissues is done.

Hormone receptor-positive breast cancers:

Once the biopsy report confirms expression of certain hormone receptors like Estrogen-positive, Progesterone-positive , Both estrogen and progesterone receptor positive breast cancers , HER2 receptor positive breast cancers and triple negative breast cancers treatment options are decided by the clinicians.

Treatments for breast cancer are determined by their types, grade, stage , overall health, age and personal preference of patient.

Early diagnosis of breast cancer is the key to successful treatment of breast cancer and a better treatment outcome:

If a patient feels the following signs and symptoms , she should immediately seek medical attention. A woman who is at higher risk of developing breast cancer must undergo breast cancer screening at regular intervals. Awareness about breast cancer is extremely necessary in underdeveloped countries and developing nations. Many precious lives can be saved with a timely diagnosis of breast cancer.

Signs and symptoms of breast cancer:

There are no generalized symptoms of breast cancer and they may differ from patient to patient. The most common breast cancer signs and symptoms are as follows:

  • Thickening or swelling on any part of breast
  • Redness or flakiness of skin on breast , nipple or areola
  • Dimpling of breast skin
  • Any unusual breast discharge ( other than milk ) including blood
  • sinking in of nipple ( nipple turning inwards) or pain in the nipple area

Chemotherapy for breast cancer treatment in India:

Chemotherapy is administering of chemo. medicines for killing the breast cancer cells. Chemotherapy for breast cancer can be given either prior to surgery to reduce the size of tumor and when many lymph nodes are involved to reduce the extent of surgery. Chemotherapy given prior to breast cancer surgery is called neoadjuvant chemotherapy for breast cancer treatment.

Chemotherapy is also given in some breast cancer patients post the breast cancer surgery to kill any leftover cancerous cells or any cancer cells that may have escaped to lymphatic system or blood vessels. When chemo is given post the surgery , it is called adjuvant chemotherapy for breast cancer treatment.

In metastatic breast cancers chemotherapy is the mainstay of breast cancer treatments.

Duration and type of chemotherapy depends on the type of breast cancer and how the patient responds to a chemotherapy regimen for breast cancer treatment.

Cost of breast cancer chemotherapy in India:

Cost will depend on mode of administration of chemotherapy and the chemotherapy regimen and chemotherapy medicines used.

Cost of putting Chemo port for breast cancer chemotherapy:

Cost of one cycle of Chemotherapy ( Depending on chemotherapy medicine & it's make )400 -1200 USD

Side effects of breast cancer chemotherapy:

Most side effects of chemotherapy for treating breast cancers are temporary and subsides once the chemotherapy is over. They may be worse during the treatment and may last for some weeks or month following the chemotherapy. Rarely they are long lasting and permanent.

Chemotherapy for breast cancers is given to kill fast-multiplying breast cancer cells but at times they do affect fast-multiplying healthy cells too like those in hair follicles , bone marrow and digestive tract.

Side effects of chemotherapy usually occur because of damage to these fast multiplying healthy cells but the healthy cells are capable of repairing the chemotherapy damage once the chemotherapy cycles are finished.

Common and short term side effects of breast cancer chemotherapy are listed below:

  • Hair loss ( Falling of hair ). Hair regrows once chemotherapy is over.
  • Nausea and vomiting ( Your medical oncologist may prescribe anti-emetic medicines to reduce severity ).
  • Loss of appetite
  • Constipation or diarrhea ( Your chemotherapy doctors will manage it with medicines).
  • Skin and change of nail colour
  • Generalized fatigue or low energy levels
  • Mouth sores
  • Because of myelosuppression ( Damage to bone marrow cells - immunosuppression) , low numbers of white blood cells thus increased risk of infections.
  • Nerve damage ( Chemotherapy induced neuropathy )
  • Chemo-brain ( loss of cognitive function that affects memory and concentration).

Rare but long term side effects of breast cancer chemotherapy:

  • Chemotherapy induced infertility. some anti cancer medicines ( Chemotherapy ) hampers ovarian function and ovarian reserves. Although it is rare but can affect the chances of pregnancy for younger women. If you want to start a family after breast cancer treatment discuss this possibility with your medical oncologist. Preservation of ovaries and eggs prior to chemotherapy helps in achieving pregnancy through in vitro fertilization in such breast cancer patients.
  • Chemotherapy induced bone mass loss. As Chemotherapy hampers ovarian function , many women undergoing chemotherapy for breast cancer treatment may experience an early onset of menopause. and as a consequence may suffer from osteopenia or osteoporosis. The chances of fractures increases in such women. You may need to do periodic bone density tests and may require to take treatments for further bone loss.
  • Chemotherapy induced Heart Damage. Chemotherapy for breast cancer carry a small risk of weakening of the heart muscles and other heart problems.
  • Increase risk of leukemia. Breast cancer chemotherapy also increases chances of a secondary cancer like that of blood cancer after many years of breast cancer treatment.

These proteins helps breast cancer cells to thrive , multiply and live longer. These proteins are expressed noticeably in pathological findings of breast cancer tissues send for biopsy.

Radiation-therapy for treating breast cancer :

Radiation therapy for treatment of breast cancer is used for reducing the chances of recurrence of breast cancer. Radiation therapy is also used for providing relief from pain

After careful evaluation of breast cancer size, stage, and location  a radiation oncologist may give options of doing either more commonly used external beam radiation or brachytherapy for treating breast cancer.

External beam radiation therapy for breast cancer treatment :

External beam radiation therapy for treatment of breast cancer is targeting focused high energy X-rays or subatomic particles like protons and photons on breast cancer cells to destroy them. Most common external beam radiation techniques for treating breast cancer are:

  • 3D conformal radiation therapy involves taking 3 dimensional pictures of tumor with help of a computer. It is also named as 3D CRT. 3D CRT helps the treating radiation oncologist give highest dose of radiation to tumor and sparing the normal healthy tissues.
  • Volume modulated arc therapy ( VMAT) is a newer form of external beam radiation therapy for treating breast cancers. In VMAT for breast cancer radiation therapy the machine rotates around patient's body while she is lying down, delivering continuous doses of radiation toward the breast tumor. During VMAT, patient receive customized doses of radiation as a machine encircles patient's body in one or more rotations, or arcs. VMAT allows the radiation oncologists to direct high doses of radiation toward breast tumors without exposing the surrounding healthy tissue to harmful amounts of radiation.
  • Image modulated radiation therapy ( IMRT) uses a computer controlled CT scan for delivering radiation beams at different doses and angles to breast cancer tissues. IMRT helps the radiation oncologist to target breast cancer tissues without harming critical organs like lungs, heart and surrounding normal tissues.
  • Image guided radiation therapy ( IGRT) is a new technique of Image modulated radiation therapy with daily imaging of radiation area for treating breast cancer. IGRT uses imaging technologies such as PETMRI, and CT to more accurately and safely deliver radiation to cancer cells. The machines that are used to deliver the radiation, called linear accelerators, are equipped with the imaging equipment so that doctors can confirm exactly where the tumor is in the body before and during treatments.

Internal radiation therapy (Brachytherapy) for breast cancer treatment:

Internal radiation therapy ( brachytherapy ) is placing a device containing radioactive material inside the body of breast cancer patient either at the tumor bed or nearby it. Brachytherapy could be a good option for certain women who have undergone lumpectomy for breast cancer treatment. Brachytherapy is not an option for other breast cancer patient as suitability depends on size and location of tumor.

Radiation therapy for breast cancer treatment is done after the healing of the surgical wound and with the intention of reducing chances of recurrence. We are associated with one of the best and most experienced radiation oncologist for breast cancer treatment in India.

Cost of Radiation therapy for Breast cancer treatment:

Cost depends on type and number of radiation therapy sessions decided by the Radiation oncologist.

Cost of Radiation therapy for breast cancer treatment in India.2400-6000 USD

Hormone therapy for breast cancer treatment in India:

The two main female hormones estrogen and progesterone are produced by ovaries in women who are still menstruating. Estrogen and progesterone are also produced by certain fatty tissues and skin in both pre menopausal and post menopausal women and men too.

In women estrogen is responsible for development and maintenance of female sex characteristics and the growth of long bones while progesterone plays a role on regulating the menstrual cycle and pregnancy.

Hormone estrogen and progesterone are also identified as helping factors in development of certain breast cancers called hormone positive breast cancers.

To know if breast cancer in a patient is hormone positive , a doctor sends the tissues of breast cancer obtained through biopsy. The hormone positive breast cancers are identified through expression of hormone receptors from the pathological findings of biopsy of breast cancer tissues. If the tissues show estrogen receptor the cancer is termed as ER positive breast cancer (ER+), If Progesterone receptor is more prominent , breast cancer is termed as PR+ . If both estrogen and progesterone receptors are prominent in breast cancer tissues , breast tumor is termed as ER-PR+ or hormone receptor positive ( HR +) breast cancers. If the breast cancer tissues does not show either estrogen receptors nor progesterone receptors they are termed as HR- ( hormone receptor negative ) breast cancers.

Hormonal treatment for HR+ breast cancers slows or stops the growth of hormone-sensitive tumors by blocking the body’s ability to produce hormones or by interfering with effects of hormones on breast cancer cells.

Side effects of Hormone therapy for treatment of breast cancers:

Side effects of hormonal therapy for treating breast cancer depends on which medicine is used . The common side effects of hormonal therapy for breast cancers are:

  • Hot flushes
  • night sweats
  • Vaginal dryness
  • Disruption if menstrual cycles in pre menopausal women.

The less common but serious side effects of hormone therapy for treatment of breast cancers may include:

  • Risk of blood clots in legs and lungs
  • Increased risk of Strokes
  • Increased risk of developing cataract
  • Endometrial cancer or uterine sarcoma
  • Bone loss in pre menopausal women
  • Mood swings , depression and loss of libido.

The benefits and harms of taking hormone therapy should be carefully weighed for every Individual HR+ breast cancer patient..
